经腋下直切口的开胸术

目的：探讨经腋下直切口开胸术的应用及其优缺点。方法：用腋下直切口对４５例不同胸内疾病患者进行多种手术。结果：这种切口能保护肌肉，对胸内结构显露良好，操作简单省时，术后疼痛轻，肩部活动不受明显影响，疤痕短而隐蔽。结论：可有选择地采用腋下直切口取代标准的后外侧切口进行开胸手术。在上部胸壁部分切除、困难的上叶肺切除或肺门部粘连紧密时，不宜用这种切口。

Vertical Axillary Muscle Sparing Thoracotomy

Objective:To explore the advantages and disadvantages of vertical axillary muscle sparing thoracotomy.Methods: Forty five patients were operated on with this approach for various intrathoracic lesions,and the operation results were analysed.Results:The technique was simple and quick.Postoperatively,this procedure was associated with less pain,less limitation of shoulder girdle movement and excellent cosmetic healing.Conclusions: This approach offers an alternative to the standard muscle cutting posterolateral thoracotomy for elective procedures.When chest wall resections or extremely difficult upper lobectomy or hilar dissections are anticipated,this approach is not suggested.
