骨盆骨折伴血流动力学不稳定患者的外固定治疗

摘    要:目的探讨对骨盆骨折伴血流动力学不稳定患者开展外固定治疗的临床效果。方法选取台州市第一人民医院2011年10月—2013年10月收治的骨盆骨折伴血流动力学不稳定的患者63例,其中33例给予骨盆外固定支架治疗的患者作为观察组,30例未给予骨盆外固定治疗的患者作为对照组,观察2组患者的临床治疗效果与并发症情况,并进行比较分析。结果观察组1例因严重颅脑损伤致死,对照组3例因失血量过多致死。观察组患者的病死率显著低于对照组,组间比较差异具有统计学意义(P〈0.05)。对照组患者纠正率为70.0%,观察组患者纠正率为90.9%,观察组显著好于对照组(P〈0.05)。对照组患者的ISS评分为11.7±1.9,观察组患者的ISS评分为9.8±0.7,观察组显著好于对照组(P〈0.05)。结论骨盆骨折伴血液动力学不稳定患者经常会发生无法控制的出血现象,经常会引发严重休克或大出血,这也是此病患者病死率高的主要原因。针对该类患者,在临床治疗时应该及时采取外固定支架的相关治疗,可以有效控制骨盆容积,降低断端的出血总量,具有确切的临床应用价值。

External fixation treatment of pelvic fracture patients with hemodynamic instability

Abstract:Objective To study the clinical efficacy of external fixation treatment for pelvic fracture patients with hemodynamic instability. Methods A total of 63 patients diagnosed as pelvic fracture and hemodynamic instability in our hospital from October 2011 to October 2013 were enrolled and divided into the treatment group( pelvic external fixator,n =33),and the control group(common treatment,n = 30),then the clinical efficacy and the incidence of complication between the two groups were compared. Results One patient died of severe craniocerebral injury in the treatment group,while 3 patients died of too much blood loss in the control group,the mortality rate of the treatment group was significantly lower than that of the control group(P〈0. 05);The correct rate of the treatment group was 90% which was significantly higher than that of the treatment group 70. 0%(P〈0. 05). ISS score in the treatment group was 11. 7 ± 1. 9,which was significantly higher than 9. 8 ± 0. 7 in the control group(P〈0. 05). Conclusion The pelvic external fixator was an effective method for patients with pelvic fracture and hemodynamic instability,which could significantly decrease the blood loss rate,and have exact clinical application value.
