围生儿死因的临床与病理对照分析(附141例报告)

为了解围生儿死亡原因及其相关因素,对141例围生儿临床与尸检的病理资料进行对照分析。发现围生儿病理解剖前5位死因依次为致死性畸形、出血性疾病、肺透明膜病、感染性疾病、窒息;临床前5位死因依次为窒息、致死性畸形、感染性疾病、硬肿症、出血性疾病,临床与病理诊断顺位显著不一致(P<0.01)。孕龄<37周,体质量<2500 g的早产儿死因以窒息、肺透明膜病、颅内出血、肺出血为多;死胎死产的主要原因为致死性畸形和窒息。羊水过多胎儿畸形的发生率高,羊水过少窒息的发生率高。分析认为做好围生期管理,提高产前诊断,及时采取相应措施,对优生优育至关重要。

Clinical and pathologic control analysis of causes for 114 perinatal death

The purpose of this paper is to explore the main causes of perinatal death. The clinical and pathologic data of 114 cases were analyzed. The leading cause of perinatal death was fatal malformations. The 5 leading causes of perinatal death in pathologic dissection were fatal malformations, hemorrhagic diseases, hyaline membrane disease, infectious diseases, and asphyxia. The 5 leading clinical causes of perinatal death were asphyxia, fatal malformations, infectious diseases, sclerede-ma, and hemorrhagic diseases. The order of clinical and pathologic diagnosis was remarkably different (P<0. 01). The main causes of premature death of gestational age<37 weeks and body mass <2 500 g were asphyxia, hyaline membrane disease, intracranial hemorrhage, pneumorrhagia. The main causes of fetal death and dead birth were fatal malformations and asphyxi-a. The incidence of fetus malformation was higher in hydramnios and the incidence of asphyxia was higher in oligohydram-nios. Favorable perinatal management, antenatal diagnosis and corresponding measures are very important for high quality birth.
