白介素-17和细胞粘附分子-1与慢性阻塞性肺疾病相关性的研究

摘    要:目的探讨白介素-17(IL-17)和细胞粘附分子-1(ICAM-1)在慢性阻塞性肺疾病(COPD)发病中的作用及二者的相互关系。方法观察对象为COPD急性加重期住院患者(无其它炎症性疾病以及重要器官功能衰竭)40例,其中吸烟者20例,非吸烟者20例,缓解期患者40例,同期健康体检对象40人,无吸烟史。采用酶联免疫吸附法(E LI SA)对各组COPD患者的血清IL-17和ICAM-1含量进行测定,并同步测定外周血中性粒细胞计数及动脉血氧分压。结果 (1)COPD急性加重期组和缓解期组血清IL-17水平均高于健康对照组,且急性加重期组血清IL-17水平高于缓解期组(P均〈0.05)。(2)COPD急性加重期和缓解期组血清ICAM-1水平均高于健康对照组,且急性加重期组血清ICAM-1水平高于缓解期组(P均〈0.05)。(3)急性加重期吸烟组患者血清IL-17、ICAM-l水平高于非吸烟组(P〈0.05)。(4)COPD急性加重期组和缓解期组血清IL-17与ICAM-l水平均呈正相关(r值分别为0.518、0.507,P均〈0.05),且在急性加重期这两个因子与外周血中性粒细胞计数均呈正相关(r值分别为0.634,0.367,P均〈0.05),与PaO2呈负相关(r值分别为-0.676,-0.366,P均〈0.05)。结论 (1)IL-17和ICAM-1可能参与了COPD发病过程且与疾病分期及缺氧严重程度相关。(2)IL-17可能促进ICAM-1的表达,二者均与COPD全身炎症相关。(3)吸烟是促进COPD患者IL-17和ICAM-1表达的重要因素。

Abstract:Objective To study the changes of interleukin(IL)-17 and InterCellular adhesion molecule(ICAM)-1 in serum of patients with chronic obstructive pulmonary disease(COPD) during both acute exacerbation and stable stage and their relationship with each other.Methods 120 subjects were selected randomly and divided them into normal control group,COPD group during acute exacerbation and COPD group of remission.Patients with COPD during acute exacerbation were further divided into smoking and non-smoking group.The concentrations of IL-17 and ICAM-1 were measured respectively by ELISA.At the same time,the number of neutrophil in peripheral blood of all subjects was counted and the PaO2 was measured by blood gas analysis.Results The concentrations of IL-17 and ICAM-1 of COPD patients were obviously higher than those of healthy volunteers(P0.05),and the concentrations of IL-17 and ICAM-1 were higher in COPD patients during acute exacerbation than in those at stable stage(P0.05).The concentrations of IL-17 and ICAM-1 of smoking group were higher than those of nonsmoking group during acute exacerbation(P0.05).There was a positive correlation between the concentrations of IL-17 and the concentrations of ICAM-1 in patients with COPD during acute exacerbation and at stable stage.Positive relationship were shown between serum IL-17 and ICAM-1 and blood neutrophil while negative relationship demonstrated between the levels of cytokines and PaO2 during acute exacerbation.Conclusion The concentrations of IL-17 and ICAM-1 of COPD patients were obviously higher than those of healthy volunteers(P0.05),and the concentrations of IL-17 and ICAM-1 were higher in COPD patients during acute exacerbation than in those at stable stage(P0.05).The concentrations of IL-17 and ICAM-1 of smoking group were higher than those of nonsmoking group during acute exacerbation(P0.05).There was a positive correlation between the concentrations of IL-17 and the concentrations of ICAM-1 in patients with COPD during acute exacerbation and at stable stage.Positive relationship were shown between serum IL-17 and ICAM-1 and blood neutrophil while negative relationship demonstrated between the levels of cytokines and PaO2 during acute exacerbation.
