白内障超声乳化术对单纯老年性白内障患者与合并糖尿病患者泪膜影响的比较

摘    要:目的观察白内障超声乳化术对单纯老年性白内障患者与合并糖尿病患者泪膜的影响。方法将超声乳化白内障术患者67例(76只眼)分成两组。A组为单纯患老年性白内障组35例(40只眼);B组为合并患糖尿病组32例(36只眼),分别于术前和术后1、7、30、90天观察患者干眼症状、泪膜破裂时间(BUT)、角膜荧光素染色及下睑中央泪河高度。结果与术前相比,干眼症状在术后1、7天增加,泪膜破裂时间在术后1、7、30天明显缩短,角膜荧光素染色在术后1、7天明显增高(P<0.05)。比较两组术后干眼发病率有显著差异(P<0.05)。结论超声乳化白内障吸出术可影响泪膜的稳定性,尤其是合并糖尿病患者泪膜功能较差,术后恢复也缓慢。

Abstract:OBJECTIVE To observe the changes of tear film after ultrasonic phacoemulsification between simple age-related cataract and diabetic cataract.METHODS Seventy-six cases(76 eyes)that underwent ultrasonic phacoemulsification were divided into 2 groups.Group A: 35 cases(40 eyes)with simple age-related cataract.Group B:32cases(36 eyes)with diabetic cataract.The subjective complains of dry eye,tear break-up time,corneal fluorescein staining and the height of tear meniscus were measured preoperatively and 1 day,7,30 and 90 days postoperatively.RESULTS At 1 day,7days postoperatively,the subjective complains of dry and eye corneal fluorescein staining increased significantly(P<0.05).At 1day,7 and 30 days postoperatively,tear break-up time reduced greatly((P<0.05)).Dry eye risks were higher in group B.CONCLUSIONS Ultrasonic phacoemulsification significantly alters tear film of all patients in a short term postoperatively.The tear film stability of diabetic cataract reduced in early stage after phacoemulsification and recovered more slowly than that of age related cataract.
