乳管镜定位超声引导下乳管内病灶真空辅助空芯针微创旋切术

目的评价乳管镜定位超声引导下真空辅助空芯针微创旋切乳管内病灶的临床应用价值。方法回顾性分析19例在乳管镜下发现乳管内隆起性病灶的乳头溢液患者,在乳管镜定位联合超声引导下,采用真空辅助空芯针微创旋切乳管内病变。结果 19例患者均找到乳头状病灶,导管内癌2例,导管内乳头状瘤16例,乳头状瘤病2例,导管内乳头状增生1例。肿瘤切除准确率达100%。结论乳管镜定位联合超声引导下真空辅助空芯针微创旋切手术定位准确,组织损伤小,病理取材方便,术后美观效果好,是目前乳管内病灶切除的可靠方法。

MINIMALLY INVASIVE VACUUM- ASSISTED BIOPSY GUIDED BY FIBEROPTIC DUCTOSCOPY AND ULTRASOUND FOR MANAGEMENT OF INTRADUCTAL PROTRUSION LESIONS

Objective To evaluate the clinical application value of minimally invasive vacuum- assisted biopsy under the guidance of fiberoptic ductoscopy（ FDS） and ultrasound for management of intraductal lesions in the breast. Methods 19 cases of intraductal protrusion lesions with nipple discharge undergoing biopsy by FDS were retrospectively analysed. Intraductal lesions of the breast were removed by minimally invasive vacuum- assisted biopsy guided by FDS and ultrasound. Results Papillary lesions in 19 cases,ductal carcinoma in 2 cases,intraductal papilloma in 14 cases,papillomatosis in 2 cases and intraductal papillary hyperplasia in 1 case were all successfully removed. Conclusion Minimally invasive vacuum- assisted biopsy guided by FDS combined with ultrasound is characterized by an accurate location and minimal tissue damage,which is convenient for pathological materials and good postoperative appearances. It has been a feasible method for the removal of intraductal lesions in the breast currently.
