尺动脉腕上皮支逆行筋膜皮瓣修复手部软组织缺损

目的:探讨尺动脉腕上皮支逆行筋膜皮瓣修复手部软组织缺损的临床意义。方法:选择18例手部软组织缺损患者,皮瓣切取面积12 cm×8~6 cm×5 cm,应用尺动脉腕上皮支逆行筋膜皮瓣修复,术后观察皮瓣的血运、色泽、质地及感觉的恢复。结果:术后18例全部成活,随访3个月~3年,皮瓣的色泽、质地均满意,吻合神经的16例两点辨别觉为10~15 mm。供区创面一期修复愈合,无功能障碍。结论:尺动脉腕上皮支上行支血管解剖恒定,皮瓣厚薄适中,操作简便,是修复手部软组织缺损的较好方法。

The reconstruction of soft tissue defect of hand with a dorso-ulnar pedicled flap based on dorsal carpal branch of ulnar artery

Objective To introduce and evaluate a novel method of repairing soft tissue defect of the hand with a dorso-ulnar pedicled flap based on the dorsal carpal branch of the ulnar artery.Method Eighteen patients with soft tissue defects of hands were treated with the above mentioned flap.The size of the flap ranged from 12 cm×8 cm to 6 cm×5 cm.Results All flaps survived.The patients were followed up from 3 months to 3 years.The appearance and texture of the flaps were good.The two point discrimination of 16 flaps with neuroreconstruction was 10~15 mm.The donor sites were repaired primarily without functional impairment.Conclusion The dorsal carpal branch of the ulnar artery has consistent anatomy.The flap based on this branch is thin and easy to harvest,and excellent to repair small or medium soft tissue defect of hand.
