早期声门型喉癌患者放疗的临床疗效及预后影响因素分析

目的探讨放疗在早期声门型喉癌患者治疗中的临床效果及预后影响因素。方法将110例早期声门型喉癌患者随机分为2组,对照组应用垂直半喉切除手术干预,研究组应用放射治疗,对比2组喉功能、并发症、复发情况,并对研究组患者预后影响因素进行分析。结果研究组喉功能好于对照组(P<0.05);研究组并发症及复发情况低于对照组(P<0.05)。经单因素分析年龄、性别、临床分期以及淋巴结转移均为影响患者复发的危险因素(P<0.05);预后多因素分析结果发现,患者年龄、性别、临床分期及淋巴结转移是影响患者预后的主要因素(P<0.05)。结论早期声门型喉癌患者临床干预过程中使用放疗效果较为满意,可有效提高患者喉功能,减少并发症及复发的发生。预后与患者年龄、性别、临床分期及淋巴结转移有关。

Clinical Effect and Prognostic Factors of Radiotherapy in Patients with Early Glottic Carcinoma

Objective To investigate the clinical effects and prognostic factors of radiotherapy in patients with early glottic carcinoma.Methods 110 cases of early glottic cancer were randomly divided into 2 groups,the control group used vertical partial laryngectomy for surgical intervention,the study group used radiotherapy,comparison of laryngeal function,complications,recurrence,prognosis and the analysis of influencing factors were conducted.Results Laryngeal function in the study group was better than the control group(P<0.05);complications and recurrence in the study group were lower than those of the control group(P<0.05);Age,gender,clinical stage and lymph node metastasis were all risk factors affecting patients'recurrence(P<0.05).The results of multivariate analysis of prognosis showed that age,gender,clinical stage and lymph node metastasis of patients were the main factors affecting prognosis of patients(P<0.05).Conclusion Early clinical intervention of glottic cancer patients during radiotherapy has satisfactory effect,can effectively improve the prognosis of patients with laryngeal function,reduce complications and recurrence.Prognostic factors are related with age,gender,clinical stage,and lymph node metastasis.
