地佐辛、依托咪酯复合丙泊酚麻醉在老年患者无痛胃镜术中的应用

目的:观察地佐辛、依托咪酯复合丙泊酚麻醉在老年患者无痛胃镜术中的临床效果和安全性。方法:选取100例无痛胃镜检查的老年患者,年龄65~75岁,ASAⅠ~Ⅱ级,随机分为两组:Ⅰ组芬太尼0.05 mg+丙泊酚0.6 mg/kg+依托咪酯0.1 mg/kg,Ⅱ组地佐辛5 mg(无痛胃镜前5 min给予)+丙泊酚0.6 mg/kg+依托咪酯0.1 mg/kg。分别记录给药前、睫毛反射消失时和恢复意识时的平均动脉压、心率、呼吸频率、脉搏饱和度;统计不良反应、苏醒时间。结果:Ⅱ组患者各时间点MAP、HR及SPO2优于I组,差异有统计学意义(P<0.05);Ⅱ组患者呼吸抑制、体动等不良反应的发生率优于I组(P<0.05)。结论:地佐辛、依托咪酯复合丙泊酚在老年患者无痛胃镜术中是一种安全有效的麻醉方法。

Application of dezocine etomidate combined with propofol in anesthesia for painless gastroscopy in elderly patients

Objective： To evaluate clinical effect and safety of dezocine, etomidate combined with propofol for painless gastroscopy in elderly patients. Methods ： 100 elderly patients examinated by painless gastroscopy, aged 65 to 75, ASA I -Ⅱ level, were randomly divided into two groups： group I was given fentanyl 0.05 mg ＋ propofol 0.6 mg/kg ＋ etomidate 0.1 mg/kg, group Ⅱ was given dezocine 5 mg （5 rain before give painless gastroscopy） ＋ propofol 0.6 mg/kg ＋ etomidate 0.1 mg/kg. Mean arterial pressure, heart rate, breathing rate and pulse saturation were recorded before administration, the loss of eyelash reflex and restoration of consciousness. Adverse ractions and awakening time were analyzed. Results： MAP, HR, SPO2 at the time point of group Ⅱ were better than group I, the difference was statistically significant （P 〈 0.05）. group Ⅱ of patients the incidence of adverse reactions such as respiratory depression and bodymovement was better than group I（P 〈 0.05）. Conclusion： Application of dezocine, etomidate combined with propofol for painless gastroscopy in elderly patients is safe and effective.
