危重患儿血D-二聚体、C-反应蛋白检测的临床意义

目的 探讨危重患儿血D-二聚体及C-反应蛋白（CRP）的变化与病情危重程度的关系.方法 按照小儿危重病例评分（PCIS）将患儿分为非危重组、危重组和极危重组,采用免疫比浊法分别测定患儿血中D-二聚体和CRP水平,统计分析其水平变化的临床意义.结果 随着PCIS分值降低,患儿病情加重,D-二聚体与CRP含量增加；不同组间D-二聚体及CRP比较,差异均有统计学意义.PCIS与D-二聚体、CRP呈负相关,相关系数分别为-0.368,-0.413,P均＜0.01.结论 D-二聚体和CRP水平一定程度上体现患儿的病情危重程度,两者在血中的水平与PCIS呈负相关,可作为早期识别危重症患儿及评估危重程度的指标.

Clinical significance of detecting blood D-dimer and C-reactive protein in critically-ill children

Objective To assess the severity of the illness by means of the D-dimer and C-reactive protein （CRP） levels in the critically-ill children＇s blood. Methods Patients were classified into three groups in accordance with the pediatric critical illness score （PCIS）. D-dimer and CRP levels in the blood of children were measured by immunoturbidimetry respectively. The data were analyzed to explore the clinical significance. Resalts In these critically-ill children, levels of D-dimer and CRP increased when the PCIS score decreased and their disease condition became aggravated. There were significant differences between the test values of these three groups. Moreover, PCIS was negatively correlated with the level of D-dimer and CRP. The correlation coefficient was -0. 368 （P〈0.01） and -0. 413 （P〈0.01）, respectively. Conclusion The measurement of blood D-dimer and CRP represents severity of the illness in a certain degree. They are negatively related to PCIS, which may be useful for identification of critically ill children and assessment of severity of the illness in early stage.
