摘    要:目的分析3D玻璃体超声成像对剖宫产瘢痕妊娠(cesarean scar pregnancy,CSP)分型诊断效能和Vocal能量血流参数对CSP诊断价值。方法选取CSP 176例作为研究组,同期剖宫产后正常妊娠94例作为对照组,分析3D玻璃体超声成像对CSP分型的诊断效能,并评估Vocal能量血流参数血管化指数(VI)、血流指数(FI)和血管化血流指数(VFI)对CSP的诊断价值。结果CSP 176例3D玻璃体超声成像诊断160例,3D玻璃体超声成像对CSP诊断的灵敏度为90.91%,特异度为89.30%。3D玻璃体超声成像和CSP术后诊断160例CSP分型的诊断效能具有一致性(Kappa=0.755)。研究组Vocal能量血流参数VI、FI和VFI均明显高于对照组,差异有统计学意义(P<0.01)。受试者工作特征曲线分析结果显示,Vocal能量血流参数VI、FI和VFI对CSP具有一定的诊断价值(P<0.01)。结论3D玻璃体超声成像对CSP分型的诊断效能较高;Vocal能量血流参数VI、FI和VFI对CSP具有一定的诊断价值。

Abstract:Objective To analyze the diagnostic efficiency of 3D-Glassbody ultrasonic imaging for cesarean scar pregnancy(CSP)classification and the value of Vocal energy blood flow parameters in the diagnosis of CSP.Methods A total of 176 cases of CSP were selected as the research group,and 94 cases of normal pregnancy after cesarean section during the same period were selected as the control group.The diagnostic efficiency of 3D-Glassbody ultrasonic imaging for CSP classification was analyzed,and the value of Vocal energy blood flow parameters,including vascularization index(VI),blood flow index(FI)and vascularized blood flow index(VFI)for the diagnosis of CSP was evaluated.Results Of the 176 patients with CSP,160 were positive for 3D-Glassbody ultrasonic imaging.The sensitivity of 3D-Glassbody ultrasonic imaging for CSP diagnosis was 90.91%and the specificity was 89.30%.The diagnostic efficacy of 3D-Glassbody ultrasonic imaging was consistent with CSP postoperative diagnosis of 160 CSP classification(Kappa=0.755).The Vocal energy blood flow parameters VI,FI and VFI of the research group were significantly higher than those of the control group,and the difference was statistically significant(P<0.01).The results of receiver operating characteristic curve analysis showed that the Vocal energy blood flow parameters VI,FI and VFI had a certain diagnostic value for CSP(P<0.01).Conclusion 3D-Glassbody ultrasonic imaging has high diagnostic efficiency for CSP classification,and Vocal energy blood flow parameters VI,FI and VFI have certain diagnostic value for CSP.
