胸乳入路腔镜手术治疗甲状腺良性肿瘤的临床应用研究

目的 分析胸乳入路腔镜手术治疗甲状腺良性肿瘤的临床应用。方法 选择2018 年2 月～2020 年2 月我院收治的甲状腺良性肿瘤患者120 例，按照计算机随机分组法将患者分为两组，每组各60 例，对照组入院后实施传统开放手术治疗，观察组入院后实施胸乳入路腔镜手术治疗，比较两组患者的临床指标，术后并发症发生率及手术前后的血清应激水平。结果 观察组患者的临床指标（手术操作时间、术中失血量、术后引流量、平均住院时间）优于对照组（P＜0.05）；观察组患者术后并发症总发生率为5.0%，对照组患者术后并发症总发生率为13.3%，差异有统计学意义（P＜0.05）；两组患者手术前的血清应激水平比较，差异无统计学意义（P>0.05）；两组患者手术后的血清应激水平均有一定变化，观察组患者手术后的血清应激水平优于对照组（P＜0.05）。结论 经胸乳入路腔镜手术可有效治疗甲状腺良性肿瘤，患者的临床情况更佳，术后康复速度更快，预后情况更好，血清应激水平变化幅度更小，值得应用。

Research on the clinical application of endoscopic surgery via breast approach in the treatment of benign thyroid tumors

Objective To analyze the clinical application of endoscopic surgery via breast approach in the treatment of benign thyroid tumors.Methods From February 2018 to February 2020,a total of 120 patients with benign thyroid tu mors admitted to our hospital were randomly divided into the control group(n=60) and the observation group(n=60) by computer.The control group was treated with routine open surgery after admission,while the observation group was treated with endoscopic surgery via breast approach after admission.The clinical indexes,postoperative complications rate and serum stress reaction levels before and after operation were compared between the two groups.Results The clinical indexes(operation time,intraoperative blood loss,postoperative drainage volume and average hospitalization time) of patients in the observation group were better than those in the control group(P<0.05).The total postoperative complications rate was 5.0% in the observation group,which was lower than 13.3% in the control group,with statistically significant difference(P<0.05).There was no statistically significant difference in serum stress reaction level between the two groups before operation(P>0.05).The serum stress reaction level of patients in both groups changed after operation.And the serum stress reaction level of patients in the observation group was better than that in the control group(P<0.05).Conclusion The endoscopic surgery via breast approach effectively treats benign thyroid tumors,with better clinical conditions,faster postoperative rehabilitation,better prognosis and smaller changes in serum stress reaction level,which is worthy of application.
