重型颅脑损伤患者应激性溃疡风险因素分析

目的：探讨重型颅脑损伤患者并发应激性溃疡的相关因素及风险因素。方法：回顾性分析642例重型颅脑损伤患者的临床资料,采用χ2单因素检验分析重型颅脑损伤患者并发应激性溃疡的相关因素,采用COX风险比例模型分析重型颅脑损伤患者并发应激性溃疡的独立危险因素。结果：642例重型颅脑外伤患者并发应激性溃疡176例,应激性溃疡发生率为27.41%,单因素分析结果显示：颅脑损伤后应激性溃疡与颅脑损伤的严重程度、类型、脑疝与否、手术治疗与否、血糖水平及鼻饲与否具有相关关系,多因素分析结果显示：颅脑损伤的严重程度、类型是颅脑损伤后应激性溃疡的独立危险因素。结论：颅脑损伤严重程度及类型与应激性溃疡的发生有关,在重型颅脑损伤患者要综合评价及预防应激性溃疡的发生。

Risk factor analysis of stress ulcer on patients with severe craniocerebral injury

Objective：To investigate the risk and correlation factor of stress ulcer on patients with severe craniocerebral injury.Methods：642 patients with severe craniocerebral injury were analyzed with retrospective method,correlation factor of stress ulcer on severe craniocerebral injury were analyzed with χ2 test,and risk factor of stress ulcer were analyzed with COX risk scale model.Results：There were 176 patients with stress ulcer in 642 cases with severe craniocerebral injury,incidence rate of stress ulcer was 27.41%;univariate analyze showed that severity of craniocerebral injury,type of craniocerebral injury,whether or not operations,whether or not brain hernia,level of blood sugar and whether or not nasal feeding were correlation factors with stress ulcer;COX regression showed that severity and type of craniocerebral injury were independent risk factors of stress ulcer.Conclusion：Stress ulcer on patients of severe craniocerebral injury is correlative with severity and type of craniocerebral injury,comprehensive evaluation should be taken in patients with severe craniocerebral injury to prevent stress ulcer.
