经尿道手术治疗良性前列腺增生效果探析

目的 探讨经尿道手术治疗良性前列腺增生的手术方式,找出疗效最佳的治疗方式.方法 回顾性分析2009年2月至2010年2月收治的经临床确诊为小体积良性前列腺增生患者150例的临床资料,依据治疗方式分为单一行经尿道前列腺电切术(TURP)50例(TURP组),行TURP联合经尿道膀胱颈切开术(TUIBN)50例(TURP联合TUIBN组)和行TURP联合经尿道膀胱颈电切术(TURBN)50例(TURP联合TURBN组).重点观察记录三组最大尿流率(Qmax)、残余尿(PVR)和国际前列腺症状评分(IPSS)等评价指标,并进行对比.结果 三组患者术前IPSS、Qmax和PVR比较差异无统计学意义(P＞0.05),术后IPSS、Qmax和PVR比较差异有统计学意义(P＜0.05),TURP联合TUIBN组总体疗效好于TURP组,TURP联合TURBN组疗效最佳.结论 3种方法治疗小体积良性前列腺增生均有一定疗效,但TURP联合TURBN可作为最优选择方案.应依据患者患病情况和适应证等选择不同的治疗方式.

Study on effects of transurethral surgery in treating benign prostatic hyperplasia

Objective To study the operation modes of transurethral surgery in the treatment of benign prostatic hyperplasia in order to find the best one.Methods The clinical data in 150 cases of small volume benign prostatic hyperplasia in our hospital from February 2009 to February 2010 were retrospectively analyzed and the patients were randomly divided into 3 groups according to the treatment modes.50 cases received only TURP(TURP group),50 cases were given TURP+TUIBN and the rest 50 cases were given TURBN+TURP.The Qmax,PVR and IPSS scores were closely observed and recorded for comparison.Results The differences among IPSS,Qmax and PVR before surgery had no statistical significance(P>0.05),while the differences after surgery among them showed statistical significance(P<0.05).The total effect of TURP+TUIBN was better than that of single TURP.TURP+TURBN had best effects.Conclusion These 3 methods all have certain effects in the treatment of small volume benign prostatic hyperplasia,but TURP+TURBN is the best scheme.Different methods of treatment should be selected according to the patients′ situations and indications.
