中性粒细胞/淋巴细胞计数与降钙素原对肿瘤患者合并细菌感染的诊断价值

目的:探讨中性粒细胞/淋巴细胞计数(NLR)和降钙素原(PCT)对肿瘤患者合并细菌感染的预测诊断价值。方法:研究对象为380例肿瘤患者,其中感染组215例,非感染组165例。测定患者血常规和PCT水平,获取中性粒细胞和淋巴细胞计数,计算出NLR值。通过ROC曲线评价PCT和NLR以及两者联合对肿瘤患者合并细菌感染的早期诊断意义。结果:肿瘤患者感染组的NLR和PCT水平显著高于非感染组(P<0.001),ROC曲线分析结果显示,PCT曲线下面积(AUC)为0.750、检测灵敏度50.7%、特异度88.5%,NLR为0.741、88.8%、57.6%,两者联合时三个指标分别为0.823、94.4%、61.2%。结论:PCT和NLR对肿瘤患者合并细菌感染患者具有诊断价值,当两者联合检测时,检验灵敏度提高,联合检测可以成为预测肿瘤患者细菌感染生物标志物。

Diagnostic value of neutrophil-lymphocyte ratio and procalcitonin for detecting tumor patients with bacterial infection

Objective:To investigate the diagnostic value of neutrophil/lymphocyte ratio (NLR) and procalcitonin (PCT) on tumor with bacterial infection.Methods:380 cancer patients were enrolled,including 215 cancer patients with infection and 165 cancer patients with non-infection.PCT,NLR and the combination of the two biomarkers in the diagnosis of tumor infection were evaluated by receiver ROC.Results:The NLR and PCT levels in tumor patients were significantly higher than those in non-infected patients (P＜0.001).The ROC curve showed that the area under the curve (AUC) of PCT，NLR and the combination of the two biomarkers in the diagnosis of tumor infection were 0.750,0.741,0.823 respectively.The sensitivity of them were 50.7%,88.8%,94.4% and specificity were 88.5%,57.6%,61.2%,respectively.Conclusion:PCT and NLR may be helpful in the diagnosis of tumor patients with bacterial infection.PCT combined with NLR would be expected to become a biomarker to predict bacterial infection in tumor patients.
