| PFNA治疗老年人不稳定型股骨粗隆间骨折的疗效分析 |

| 摘 要： | 目的探讨改良股骨近端防旋髓内钉（PFNA）治疗老年人股骨粗隆间骨折的临床疗效。方法回顾性分析2006年1月-2008年6月收治的37例65岁以上老年不稳定型股骨粗隆间骨折患者进行PFNA治疗的临床资料。其中按AO分型，31．A2 22例，31．A3 15例。结果3例因脑血管意外等疾病死亡，5例失访，29例获得随访。随访时间为1-3．5年，平均1．6年。29例患者均获得一期骨性愈合．愈合时间为3．3～7．7个月，平均4．3个月。根据按Harris髋关节功能评分：优20例，良3例，一般4例，差2例，优良率79．3％（23／29）。3例出现钉尾近端的不匹配，其中2例出现大腿部疼痛，1例无痛；1例出现下肢深静脉血栓；1例出现髋内翻。结论对于老年人不稳定型股骨粗隆间骨折，PFNA具有操作简单、创伤小、骨折固定牢固、并发症少及术后患者可早期离床负重等优点，是一种治疗股骨粗隆间骨折较为的理想方法。
