术中超声在肝胆胰疾病手术中的诊断及应用价值

目的:探讨术中超声(Intraoperative Ultrasonography,IOUS)在肝胆胰疾病手术中的诊断及应用价值。材料与方法:对81例肝胆胰手术的患者进行术前经腹超声(Transabominal Ultrasonography,TAS)及IOUS检查,以病理诊断为标准,统计二者的诊断符合率、漏诊率、误诊率以及对恶性病变的敏感度和特异度。将TAS和IOUS诊断结果符合情况进行统计学分析。结果:所有病例共81例,其中良性50例,恶性31例。TAS、IOUS对肝胆胰疾病诊断的符合率、漏诊率、误诊率分别为63.00%、8.64%、6.17%及82.72%、1.24%、6.17%,对肝胆胰疾病恶性病变的敏感度、特异度分别为77.42%、76.00%及90.32%、84.00%。结论:IOUS较TAS有较高的诊断符合率,对恶性病变具有较高的敏感度和特异度,对肝胆胰疾病手术的操作具有重要的指导作用。

Diagnosis and Application Value of Intraoperative Ultrasonography in Hepatopancreatobiliary Operations

Purpose： To evaluate the diagnosis and application value of intraoperative ultrasonography （IOUS） in hepatopancreatobiliary operations. Materials and Methods： 81 patients with hepatopancreatobiliary diseases who were admitted for surgical operation were accepted IOUS and preoperative TAS. With the pathological diagnosis as the standard, to count the diagnosis coincidence rate, the missed diagnosis , the misdiagnosis rate as well as the sensitivity and specificity to the malignant lesions of IOUS and TAS. The diagnostic results TAS and IOUS of compliance for statistical analysis. Results： All cases were 81 patients including benign 50 cases and malignant 31cases. For hepatopancreatobiliary diseases, the diagno- sis coincidence rate, the missed diagnosis , the misdiagnosis rate were 63.00% , 8.64% , 6. 17% for TAS and 82. 72% , 1.24% , 6. 17% for IOUS, the sensitivity and specificity to the malignant lesions were 77.42%, 76.00% for TAS and 90. 32% , 84. 00% for IOUS. Conclusion： IOUS in the diagnosis of hepatobiliary pancreatic diseases has higher diagnosis coincidence rate than TAS. IOUS to the operation of hepatopancreatobiliary diseases plays an important role in guiding.
