食管间质瘤的影像学与病理学对照研究

目的探讨食管间质瘤（GIST）的影像学特征。方法分析8例经手术病理证实的食管GIST的影像学资料。结果8例中，良性1例，低级别恶性2例，高级别恶性5例，肿瘤大小0．6～6．2cm，平均5．3cm。钡餐造影表现为黏膜皱襞变平或破坏、管腔偏侧性狭窄、肿块内出现钡斑或窦道样改变；CT检查为腔外不规则肿块、肿块内多灶性低密度坏死区、明显的不均匀强化。结论螺旋CT对于食管GISTs的定位和提示诊断都具有非常高的价值，偏侧性狭窄伴溃疡、不规则腔外肿块、多灶性低密度区征象、明显的不均匀强化提示恶性，确诊有赖于病理学检查。

The Comparative Study of Imaging Findings and Clinic-pathological of esophageal stromal tumors

Objective To investigate the Imaging Findings of esophageal stromal tumors (GIST).Methods The clinical and imaging data of 8 cases esophageal GISTs were analyzed and summarized.In this group CT scan and barium exam was peiformed.Results The tumors included 1 cases benign,2 cases low grade malignant and 5 cases high grade malignant.The diameter varied from 0.6 cm to 6.2 cm,the average size was 5.3 cm.The main signs of barium meal of esophageal malignant stromal tumors included flattened or destroyed mucosa runae,partial lumen stenosis,and barium fleck and fistula in the tumor.On CT scans,the main manifestation was an extraluminal mass with multiple necrosis areas of low density.Conclusion Barium examination and helical CT scan were capable of precisely localizing the sites of esophageal GISTs and was also very helpful in establishing the diagnosis of esophageal GISTs,however,the differential diagnosis between benign and malignant was rather difficult and a definite diagnosis was primarily relied on the findings of pathological examination.
