电视纵隔镜在胸部疾病治疗中的应用

目的讨论电视纵隔镜在胸部疾病治疗中的应用价值。方法对2007年2月～2009年4月开展的8例应用纵膈镜经肋间切口纵膈肿物切除患者的资料进行回顾性分析，所有患者取令麻侧卧位，腋后线第5肋间做2cm切口，从镜管内伸入电凝吸引器分离，从纵隔镜镜身内取山肿瘤。结果手术时间50～90min，平均65min。8名患者术后均无出血，无发热，无胸腔积液，引流少，复查CT示：肿瘤切除完整，无残留，全部患者术后3d内拔除引流．术后病理结果分别为神经鞘瘤5例、神经纤维瘤3例。结论电视纵膈镜可以作为切除纵膈肿物的方法之一。

Video-mediastinoscopy in the Treatment of Thoracic Diseases

Objective To discuss the application ofvideo-mediastinoscopy in the treatment of thoracic diseases. Methods Eight cases with mediastinal mass who were undergo resection by intercostals mediastinoscopy from February,2007 to April,2005 in our division were retrospectively reviewed.The operation was performed under general anesthesia.The patients were kept at a lateral decubitus position.An incision 2.0cm in length was made at the fifth intercostals space along the posterior axillary line for introducing the video-mediastinoscope,separating with electric coagulation.The mediastinal mass was drawn out under mediastinoscope.Results The operation time was 50-90min（mean,65min）.All eight cases were without bleeding, fever or pleural of effusion and with less drainage flow.In all cases,tumors were complete excision by chest Comput ertomography（CT） and drainage tube removed in three days.Results of pathological examination ：Five cases ofneurilemmoma,three cases ofneurofibroma.Conclusion Video-mediastinoscopy is one of the effective methods for treatment to thoracic diseases.
