未婚妇女意外妊娠时的心理状况和伦理干预

目的：为了解未婚妇女意外妊娠时的心理卫生状况和影响因素,提高年轻未婚女性的生殖保健知识.方法：采用症状自评量表SCL-90和Beck抑郁问卷调查158例意外妊娠的未婚妇女,在妊娠终止前后和月经复潮后一周时的心理状况水平和抑郁状态.结果：未婚妇女在妊娠终止前、妊娠终止后和月经复潮时SCL-90各因子分与国内常模相比有显著性差异.对SCL-90分值的影响程度依次为受教育程度、年龄、对避孕知识的了解、孕次、本次妊娠时限.与妊娠终止前相比,妊娠终止后和月经复潮时的BDI分值有极显著性差异（P＜0.01）.中重度抑郁状态在妊娠终止前占65.8%,而在妊娠终止后和月经复潮时比例下降至53.8%和40.5%,差异显著（P＜0.01）,三个时段中,影响患者抑郁状态的主要因素并不完全相同.结论：意外妊娠的发生极大影响未婚女性的心理健康状况,意外妊娠的终止在一定程度上改善了患者的心理状况水平.心理状况差和抑郁状态的高发人群都集中在年轻、教育程度差、缺乏避孕知识的女性.

Psychiatric Status and Ethical Issue of Unmarried Women in Unexpected Pregnancy

Objective: To investigate the psychiatric status and find its effectede factors of unmarried women in unexpected pregnancy,and to improve the reproductive healthcare capacity of young unmarried women.Methods: Using the self-rating scale SCL-90 and Beck Depression Investigation,to investigate 158 cases before and after the pregnancy termination and after menses recovered 1 week.Results: There is a significantly difference in SCL-90 rating between the unmarried pregnancy women and normal women during the pregnancy termination and menses recovered 1 week(P<0.05).Effected factors on SCL-90 scores were educated levels,women's age,rating on contraceptive knowledge,parity and present pregnancy duration.There is a significantly difference in the BDI scores of unmarried pregnancy women during the pregnancy termination and menses recovered 1 week(P<0.05),the morbidity of moderate and severe depressive state is 65.8% before the pregnancy termination,and it drops to 53.8% and 40.5% respectively after the pregnancy termination and menses recovery.Effected factors in BDI scores are different from pregnancy termination and menses recovery.Conclusion: Unexpected pregnancy has dramatic effect on psychological status of unmarried women.The termination of pregnancy can improve women's psychiatric status to some extent.The women who would be in severe psychological condition and depressive situation are those younger, less educated,less knowing contraceptive issue.
