两种尺神经前置方法治疗肘管综合征疗效分析

目的比较采用传统尺神经皮下前置术和尺神经带血管蒂深筋膜瓣下前置术治疗中重度肘管综合征的临床疗效。方法回顾我院1999年至2006年期间收治的中重度肘管综合征76例,分别采用尺神经皮下前置术和尺神经带血管蒂深筋膜瓣下前置术,获得随访70例,其中尺神经皮下前置术28例,尺神经带血管蒂深筋膜瓣下前置术42例。结果术后随访6个月至3年(平均2.2年)。疗效评定按照张高孟评分标准,尺神经皮下前置术者优良率64.3%,尺神经带血管蒂深筋膜瓣下前置术者优良率85.7%,两组的疗效差异有统计学意义(P<0.05)。结论中重度肘管综合征患者宜行尺神经带血管蒂深筋膜瓣下前置术。

Efficacy analysis of two methods of treating cubital tunnel syndrome

Objective To compare the clinical efficacy of the patients with middle or severe cubital tunnel syndrome treated by subcutaneous anterior transposition and deep fascial flap anterior transposition with the nerve artery respectively.Methods Seventy-six cases were treated with the different methods (subcutaneous anterior tranaposition or deep fascial flap anterior transpo- sition) in my hospital from 1999 to 2006,70 cases were followed up.Among them,28 cases were treated with subcutaneous ante- rior transposition,and 42 cases with deep fascial flap anterior transposition.Results The cases were followed up from 6 months to 3 years(2.2 years in average).According to ZHANG Gao-men's scoring system,64.3% of the cases treated with subcutaneous anterior transposition were scored excellent and good,and in the group of treated with deep fascial flap anterior transposition with the nerve artery,85.7% scored excellent and good,the difference of efficacy between the two treating methods was statistically significant (P<0.05).Conclusions According to the difference of efficacy between subcutaneous anterior transposition and deep fascial flap anterior transposition with the nerve artery,it is recommended to treat patients with middle or severe cubital tunnel syndrome by deep fascial flap anterior transposition with the nerve artery.
