妊娠晚期孕妇颈围与脐动脉血流相关性分析

目的 探讨妊娠晚期孕妇颈围与脐动脉Vs/Vd比值的相关性.方法 126例妊娠晚期女性,按颈围分为颈围<34.7 cm组(n=81)和颈围≥34.7 cm组(n=45).分析妊娠晚期孕妇颈围与脐动脉Vs/Vd比值的关系.结果 颈围 ≥ 34.7cm组脐动脉Vs/Vd≥3.0检出率明显高于颈围<34.7 cm组(64.4％ vs 19.8％,χ2=25.166,P=0.000).多因素Logistic回归分析,颈围≥34.7 cm(Or=5.167,95％CI:2.138～12.491,P=0.000)是脐动脉Vs/Vd≥3.0的独立危险因素.排除合并妊娠相关疾病孕妇后,颈围 ≥ 34.7cm组Vs/Vd比值≥3.0检出率仍明显高于颈围<34.7 cm组(47.8％ vs 11.5％,χ2=14.728,P=0.000).多因素Logistic回归分析,颈围≥34.7 cm(Or=7.028, 95％CI:2.403～20.555,P=0.000)是Vs/Vd比值≥3.0的独立危险因素.结论 颈围与脐动脉Vs/Vd比值密切相关,提示肥胖与胎盘功能异常有关,即使在健康妊娠女性.

Correlation between neck circumference of pregnant woman in late trimester of pregnancy and umbilical artery blood flow

Objective To investigate the correlation between neck circumference of pregnant woman in late trimester of pregnancy and systolic/diastolic ratio (Vs/Vd) of umbilical artery.Methods One hundred and twenty-six pregnant women in late trimester of pregnancy were enrolled in the study.According to the neck circumference,these pregnant women were divided into two groups: group A (n=81) in which the pregnant women with neck circumference < 34.7cm,and group B (n=45) in which the pregnant women with neck circumference ≥34.7cm.The correlation between neck circumference of pregnant woman in late trimester of pregnancy and Vs/Vd ratio of umbilical artery was analyzed.Results The detection rate of Vs/Vd≥3.0 in group B was significantly higher than that in group A (64.4％ vs 19.8％,χ2=25.166,P<0.01).The multiple factor logistic regression analysis showed that neck circumference ≥34.7cm (r=5.167,95％,CI:2.138～12.491,P<0.01) was an independent risk factor of systolic/diastolic ratio≥3.0.After the pregnant women with pregnancy-related disease were excluded, the detection rate of Vs/Vd≥3.0 in group B was also significantly higher than that in group A (47.8％ vs 11.5％,χ2=14.728,P<0.01).The multiple factor logistic regression analysis showed that neck circumference ≥34.7cm (r=7.028, 95％CI:2.403～20.555,P<0.01) was an independent risk factor of systolic/diastolic ratio≥3.0.Conclusion The neck circumference of pregnant woman is closely correlated to Vs/Vd ratio of umbilical artery,which suggests that obesity is related with placental function abnormality, even in healthy pregnant women.
