脑利钠肽及呼吸指数评价参麦注射液对心瓣膜置换术后心肺功能的保护作用

摘    要:目的研究心瓣膜置换术后心肺功能的评价及保护方法。方法心瓣膜置换术患者60例随机分为两组,观察组(n=36)于术前3d及术后3d每日静脉滴注5%葡萄糖注射液250mL+参麦注射液45mL;对照组(n=24)仅每日静脉滴注5%葡萄糖注射液250mL。结果观察组术后对正性肌力药物的需求明显减少(P<0.05)。观察组术后机械通气时间为(16.8±4.75)h,对照组为(25.6±6.39)h,观察组通气时间较短(P<0.01)。两组术后并发症发生率相比无统计学差异(8.3%vs20.8%,P>0.05)。观察组BNP水平治疗后降幅较大(267.7±110.4)ng/Lvs(346.2±132.8)ng/L,P<0.05];观察组呼吸指数水平治疗后降幅也较大(0.49±0.14)vs(0.58±0.17),P<0.05]。结论参麦注射液对心瓣膜置换术后心肺功能具有保护作用。

Abstract:Objective To study the cardiac valve replacement after evaluation of cardiorespiratory fimction and protection. Method The 60 patients with cardiac valve replacement were randomly divided into two groups. The observation group (n=36) in the preoperative and postoperative 3d intravenous drip 3d day of 5% GS 250mL + Shenmai injection 45mL; the control group (n=24) only a day of intravenous drip of 5% GS 250mL. Results The observation group of positive inotropic drugs significantly reduced (P〈0.05). Observer Group for postoperative mechanical ventilation time (16.8±4.75) h, the control group (25.6±6.39) h, Observer Group shorter on ventilation (P〈0.01). The incidence of postoperative complications in the two groups showed no significant difference(8.3% vs 20.8%, P〉0.05). BNP level of the observation group larger decline after treatment(267.7±110.4)ng/L vs (346.2±132.8)ng/L, P〈0.05]; RI level of the observation group has a larger decline after treatment too(0.49±0.14) vs (0.58±0.17), P〈0.05]. Conclusion Shenmai injection on heart valve replacement has a protective effect of cardiorespiratory function.
