复方精油穴位按摩联合微波理疗对乳腺癌患者术后淋巴水肿临床观察

目的探讨乳腺癌患者术后患肢淋巴水肿的有效护理策略。方法选取乳腺癌术后淋巴水肿患者50例,随机分为对照组24例,试验组26例,对照组采用综合消肿理疗法(CDT),试验组采用复方精油穴位按摩联合微波理疗和CDT的复合理疗方法,分别比较2组患者干预前及干预后4周、8周,患侧手臂周径、中医症状积分及乳腺癌淋巴水肿症状体验指数(BCLE-SEI)等指标。结果干预后8周,2组患者患肢水肿程度均较干预后4周减轻(P<0.05),试验组减轻程度较对照组显著,差异有统计学意义(P<0.05);重复测量方差分析显示,试验组干预后不同时间点中医症状积分得分、患肢BCLE-SEI得分均低于同期对照组(P<0.05)。2组患者中医症状积分得分、患肢BCLE-SEI得分的时间效应、组间效应及交互作用差异均有统计学意义(P<0.05)。进一步进行组内比较,后一时间点得分均较前一时间点低,差异有统计学意义(P<0.05)。结论复方精油穴位按摩联合微波理疗对乳腺癌患者术后淋巴水肿的干预更加有效,值得临床推广、实践。

Clinical Observation on Compound Essential Oil Acupoint Massage Combined with Microwave Physiotherapy on Postoperative Lymphedema in Patients with Breast Cancer

Objective To explore the effective nursing strategy of lymphedema in the affected limb of breast cancer patients after operation.Methods Fifty patients with postoperative lymphedema of breast cancer were randomly divided into control group(24 cases)and experimental group(26 cases).The control group was treated with comprehensive detumescence therapy(CDT).The experimental group was treated with compound essential oil point massage combined with microwave therapy and CDT.The two groups of patients were compared with arm circumference,traditional Chinese medicine symptom score and breast cancer lymphedema symptom experience index(BCLE-SEI)before the intervention,4 weeks and 8 weeks after the intervention.Results Eight weeks after the intervention,the degree of edema in the two groups was significantly lower than that in the control group(P<0.05).Repeated measurement analysis of variance showed that the scores of traditional Chinese medicine symptom score and BCLE-SEI score of affected limb in the experimental group were lower than those in the control group at different time points after intervention(P<0.05).There were significant differences in traditional Chinese medicine symptom score and BCLE-SEI score of affected limb,time effect,inter group effect and interaction between the two groups(P<0.05).The score of the latter time point was lower than that of the former time point,and the difference was statistically significant(P<0.05).Conclusion Compound essential oil acupoint massage combined with microwave physiotherapy is more effective in the treatment of postoperative lymphedema of breast cancer patients,which is worthy of clinical promotion and practice.
