2型糖尿病肾病患者血胱抑素C、尿蛋白排泄与糖化血红蛋白的关系研究

摘    要:目的:观察 2 型糖尿病并发肾病患者的血清胱抑素 C(Cys-C)、尿白蛋白及糖化血红蛋白 (HbA1c) 的水平及其 3 种指标之间的相互关系.方法:选取本院 2010 年 2 月 -2011 年 4 月泌尿科住院部收治的 2 型糖尿病患者 86 例,其中并发肾病患者 47 例作为观察组,无合并肾病患者39 例作为对照组,另选取 42 例体检健康者作为空白组,患者均于空腹 12 h 后采血,收集并留取 24 h 尿,检测患者 Cys-C、24 h 尿白蛋白及 HbA1c的水平,并观察三种指标之间的相关关系.结果:三组患者血肌酐水平比较,差异无统计学意义 (P〉0.05),而三组患者 Cys-C、24 h 尿白蛋白及HbA1c 水平比较,差异具有统计学意义 (P〈0.05) ;HbA1c 与 24 h 尿白蛋白呈正相关 (r=0.482),Cys-C 与 24 h 尿白蛋白呈正相关 (r=0.658),HbA1c与 Cys-C 呈正相关 (r=0.359).结论:Cys-C、24 h 尿白蛋白及 HbA1c 的水平可作为 2 型糖尿病肾病肾功能损害早期诊断的重要指标.

Study the Relation on Serum Cystatin C,Urine Protein Cxcretion and Glyeosylated Hemoglobin in Type 2 Diabetes

Abstract:Objective :To observe the levels on serum cystatin C (Cys-C), urinary albumin and glycosylated hemoglobin (HbAlc) in Type 2 Diabetes, and explore the correlation between the three indicators.Method :86 cases with type 2diabetes from February 2010 toApril 2011 were selected, including47 cases with kidney disease as the observation group, 39 cases without kidney disease as a control group, and 42 healthy subjects as the blank group, to blood after 12 h fasting, and collected urine for 24 h, detect Cys-C, 24 h urinary albumin and HbAlc levels, and to observe the correlation between the three indices. Result :The serum creatinine levels between 3 groups were not significantly different (P〉0.05), while the Cys-C, 24 h urinary albumin and HbAlc levels were significantly different, with statistical significance (P〈0.05) ; HbAlc and 24 h urinary albumin were positively correlated (r=0.482), Cys-C and 24 h urinary albumin were positively correlated (r=0.658), HbAlc was positively correlated with Cys-C ( r=0.359).Condusion :Cys-C, 24 h urinary albumin and HbAlc levels can be used as important indicators for the early diagnosis in type 2 diabetes nephropathy patients with impaired renal function.
