多排螺旋CT在急性肠梗阻诊断中的临床价值

目的探讨多排螺旋CT(MDCT)在急性肠梗阻诊断中的价值。方法回顾性分析51例经临床、手术及病理证实为急性肠梗阻多排螺旋CT检查资料,并将临床、手术病理结果与MDCT诊断结果对照。扫描采用MDCT平扫或平扫+增强扫描,成像方法为薄层横断面、多方位重建成像。结果与临床、手术病理结果对照,多层螺旋CT诊断急性肠梗阻的敏感性为100%,部位准确率为98%,病因诊断准确率为94%。其中,肠道肿瘤20例,粘连性肠梗阻13例,肠道炎症5例,腹部疝6例,粪石性肠梗阻3例,胆石性肠梗阻2例,肠道扭转2例。结论 MDCT对急性肠梗阻、部位及其病因的诊断、血供情况的了解均有重大意义,可作为急性肠梗阻的首选检查方法。

The Clinical Value of Multidetector Computed Tomography in Diagnosing Acute Intestinal Obstruction

Objective To evaluate the value of multidetecter computed tomography（MDCT） in the diagnosis of acute intestinal obstruction.Methods MDCT findings in 51 patients with surgically or clinically be proven acute intestinal obstruction were retrospectively reviewed.Non-contrast and double phase contrast enhanced MDCT including thin slice axial scan and multiplanar reformations（MPRs） were performed in all patients.Results MDCT manifestations were consistent with clinical and surgical findings in all 51 patients.MDCT diagnostic accuracy of intestinal obstruction was 100%,the accuracy of the location was 98%,and the accuracy of the cause was 94%,including intestinal neoplasms（20cases）,adhesions（13cases）,intestinal inflammation（5cases）,abdominal hernia（6cases）,fecal stone obstruction（3 cases）,gallstone obstruction（2cases）,intestinal torsion（2cases）.Conclusion MDCT can play an important role in the diagnosis of intestinal obstruction including location,degree and causation of disease.It can also be useful in observing intestinal blood supply.It can be the first choice in diagnosis of acute intestinal obstruction.
