哮喘患儿血EOS、ECP、IgE水平和尘螨皮试的变化及其相关性研究

摘    要:目的 通过检测哮喘患儿血中EOS、ECP、IgE及尘螨皮试的水平 ,探讨其变化在儿童哮喘发生发展中的重要意义。方法 分别采用美国IMMULITE化学发光仪和德国BN10 0特种蛋白仪检测 4 3例支气管哮喘患儿和 2 5例正常对照血清ECP、IgE浓度及外周血EOS数值 ,并与尘螨皮试的变化相比较。结果 哮喘各期ECP、EOS浓度均明显高于正常对照 (P <0 0 1) ;急性发作期重度哮喘患儿ECP、EOS水平明显高于中度(P <0 0 1) ,中度哮喘患儿ECP、EOS水平高于轻度者 (P <0 0 5 ) ;EOS与ECP变化呈正相关关系 (r =0 5 34,P <0 0 1) ;而IgE及尘螨皮试结果在哮喘各期及与正常对照组比无明显差异。结论 血清ECP、EOS水平能准确地反映出哮喘患儿气道炎症的轻重程度 ,监测ECP和EOS水平对于儿童哮喘急性发作期的分度、评价抗炎治疗的效果、预测哮喘发作有重要的价值 ,而IgE测定及尘螨皮试则意义不大

Abstract:Objective To explore the roles and clinical significance of eosinophilia(EOS), eosinophil cationic protein (ECP),IgE and dust mite-skin test in children with asthma. Methods Peripheral blood amount of EOS, serum concentration of ECP and IgE were detected in 43 asthma patients and 25 controls by IMMULITE and Behring Nephelometes 100. All of the children received the skin test of dust mite extract during interview.Results Serum ECP and blood amount of EOS were significantly higher in asthma patients than those in control (P<0.01). The serum ECP and EOS were higher in patients with severe asthma than those in moderate asthma (P<0.01). The serum ECP and EOS in patients with moderate asthma were higher than that in mild asthma (P<0.05). There were positive relationship between the serum concentration of ECP and blood amount of EOS in asthma patients (r=0.534, P<0.01). There was no significant difference on serum IgE and dust mite-skin test activity between patients and controls.Conclusions Serum concentrations of ECP and blood EOS might be valuable indicators for predicting the severity of asthma in acute phase. They are also helpful in the treatment and prevention of asthma. But, serum concentrations of IgE and activity of dust mite-skin test are not so valuable as ECP and EOS.
