伽玛刀治疗原发性三叉神经痛临床随访疗效分析

目的总结分析伽玛刀治疗原发性三叉神经痛（ITN）的效果。方法对132例ITN患者进行了伽玛刀治疗，随访77例，其中早期选择三叉神经根邻近半月节处照射治疗16例，后期选择三叉神经根人脑桥处（脑桥段）照射治疗6例；选用4mm单准直器照射56例，多准直器照射21例；照射中心剂量〈70Gy27例，70-90Gy50例。结果随访3～47个月，平均25．6个月，66例（85．7％）有效，其中完全缓解41例（53．2％），部分缓解25例（32．5％）；无效11例（14．3％）；出现相关并发症（面部麻木）13例（16．9％）。结论 伽玛刀治疗1TN有良好效果，是一种安全、成熟、副作用少的治疗技术。

Analysis of Curative Effect of Gamma Knife Radiosurgery on Idiopathic Trigeminal Neuralgia

Objective To investigate the curative effect of gamma knife radiosurgery on idiopathic trigeminal neuralgia. Methods Gamma knife radiosurgery was performed in 132 patients with idiopathic trigeminal neuralgia. Seventy-seven patients were followed up from 3 to 47 months with clinical examination. The irradiated targets were at the ganglia semilunare in 16 cases and at the entry root zones into the brainstenls in 6 cases. The dose was delivered to one isocenter with the 4 mm collimator. The radiation doses were fewer than 70 Gy in 27cases, and 70-90 Gy in 50 cases. Results The following up from 3 to 47 months showed that there was complete response （no pain, hO medication） in 41 patients （53.2%, 41/77）, part response （no pain, minimal medication without side effects） in 25, and no response（unchanged pain） in 11. The total effective rate is 85.7% （66/77）. The rate of facial numb is 16.9% （13/ 77）. Conclusions The curative effect of gamme knife radiosurgery on the idiopathic trigeminal neuralgia is good. The treatment of the idiopathic trigeminal neuralgia by gamma knife radiosurgery is safe.
