尿毒症血液透析患者的卡尼汀缺乏以及左卡尼汀的补充治疗作用

探讨尿毒症血液透析患者的卡尼汀代谢并观察补充左卡尼汀的作用。方法取透析前后血测血浆游离卡尼汀浓度，研究卡尼汀与血液透析时间，肌酐清除率、食欲及透析中低血压，肌痉挛和透析后虚弱无力的关系；左卡尼汀补充３个月，观察血浆ＦＣ浓度变化和临床症状的改善。

Carnitine deficiency in hemodialysis patients and effect of levo carnitine administration

Objective:To study carnitine metabolism in hemodialysis(HD) patients and effect of L carnitine administration Methods:Blood samples were colledted from the patients before and after dialysis for determination of plasma free carnitine(FC) concentrations to decide whether they were correlated with dialysis time,creatinine clearance rate(Ccr),appetite,hypotension,muscle cramps and post dialysis asthenia or not After administration of levo carnitine for 3 month,above examinations repeated again Results:Plasma FC concentrations in HD patients were much lower than those in normal subjects( P <0 01) and were inversely correlated with dialysis time During one HD session,plasma FC concentration reduced by 70%,often companying with hypotension,muscle cramps,and post dialysis asthenia Aministration of levo carnitine can increase 4 fold plasma FC concentration and improvee clinical symptoms significantly Conclusion:Significant carnitine deficiency was found in HD patients Plasma FC concentration was elevated while carnitine related complications decreased after levo carnitine administration
