小腿皮肤缺损合并感染31例显微外科治疗分析

目的:分析研究小腿皮肤缺损合并感染的显微外科治疗方法。方法:回顾性分析2010年1月至2011年1月在我院治疗的31例小腿皮肤缺损合并感染患者的临床资料。所有患者均给予抗炎、清创、负压封闭引流,然后分别采用股前外侧皮瓣、交腿皮瓣、局部皮瓣、腓肠肌内侧头皮瓣、腓肠神经、隐神经营养血管蒂筋膜皮瓣修复。结果:全部患者创面应用VSD有效,创面新鲜,皮瓣移植修复均一期愈合。随访12周～1年,31例患者皮瓣完全成活,外形、功能满意。结论:负压封闭引流结合皮瓣移植治疗小腿皮肤缺损合并感染,全部保存了肢体,并最大限度恢复了肢体功能,临床上安全、有效、可靠。

Microsurgical treatment of 31 cases of lower leg skin defect with infection

Objective:To study the microsurgical treatment methods of lower leg skin defect with infection.Methods:A retrospective analysis was made in the clinical data of 31 patients with lower leg skin defect with infection teated in our hospital from January 2010 to January 2011.All patients accepted anti-inflammatory,debridement and VSD negative pressure drainage,then respectively used the strands of anterolateral flap,cross-leg flap,local flaps,the medial head of gastrocnemius flap,sural nerve,saphenous neurocutaneous vascular pedicle fascial flap to repair.Results:Follow up of 12 weeks~1 year,all the flap survived,the shape and function were satisfaction.Conclusion:VSD combined with skin flap transplantation are safe,effective and reliable for treatment of lower leg skin defect with infection.
