摘    要:目的探讨扩散加权成像(DWI)对前列腺增生和前列腺癌的鉴别诊断价值。方法分析18例前列腺癌和6例前列腺增生的DWI和ADC图像表现,测量癌肿区和前列腺增生区的ADC值,对两组间进行比较。结果 6例前列腺增生的中央腺体在DWI上呈不均匀稍高信号,ADC值平均为1.16。18例前列腺癌中7例位于中央腺体,在T2WI上无明确肿瘤显示。17例肿瘤在DWI上呈明显高信号,1例呈稍高信号;18个前列腺癌的平均ADC值为0.64。两组ADC值间差异有统计学意义(t=7.108,P0.01)。结论 DWI在鉴别前列腺增生和前列腺癌中具有重要价值,应作为前列腺检查的常规序列。

Evaluation of diffusion weighted imaging in differentiating prostate cancer and benign hyperplasia on 3.0T MR

Abstract:Objective To describe the appearance of benign prostate hyperplasia(BPH) and prostate cancer on diffusion weighted imaging(DWI),and to evaluate the value of DWI in differential diagnosis between two diseases.Methods DWI and ADC map findings were reviewed in 18 prostate cancer cases and 6 BPH cases.ADC values of lesions in prostate cancer and of central glands in BPH were measured and compared.Results In all of six BPH,center glands showed inhomogenous and slightly high signal intensity in DWI,and the mean value of ADC was 1.16.Among 18 cases with prostate cancer,7 tumors were located in central glands,all of which could not be identified on T2WI.On DWI,seventeen tumors showed obviously high signal intensity,and one tumor demonstrated slight high signal intensity.The mean ADC value of tumors was 0.64.There was significant difference in ADC value between two groups.Conclusion DWI has important value in differential diagnosis of BPH and prostate cancer,and should be used as convenient sequence in prostate MR imaging.
