多囊卵巢综合征外周血白细胞计数和血脂代谢的分析

目的研究多囊卵巢综合征(PCOS)患者外周血中白细胞计数和血脂代谢的变化。方法前瞻性随机在临床门诊就诊的患者中抽取82例未经药物治疗的PCOS患者,PCOS的诊断参照鹿特丹标准,体重指数(BMI)23为PCOS体重正常组,BMI≥23为PCOS超重组;对照组选择因男方无精子症在生殖中心就诊的42例非PCOS妇女。月经期或无排卵期抽取空腹静脉血,检测卵巢功能、血浆C反应蛋白(C reactive protein,CRP)、血常规、血脂代谢。结果 PCOS超重组外周血白细胞总数、中性粒细胞、淋巴细胞、单核细胞、嗜酸性粒细胞、CRP、甘油三酯(TG)、低密度脂蛋白(LDL)、胆固醇(cholesterol)、载脂蛋白B(Apo-B)明显高于PCOS正常体重组和对照组(P0.05),高密度脂蛋白(HDL)和载脂蛋白A(Apo-A)明显低于PCOS正常体重组和对照组。PCOS体重正常组CRP、淋巴细胞明显高于对照组(P0.05)。结论 PCOS患者体内存在慢性炎症,体重超重的PCOS患者表现在白细胞升高和血脂代谢异常,而体重正常的PCOS患者则表现在淋巴细胞升高。

Analysis of peripheral leukocytes and lipid metabolism of women with polycystic ovary syndrome

Objective： To investigate peripheral leukocytes count and lipid metabolism of women with polycystic ovary syndrome（PCOS） Methods. Eighty two patients with PCOS were recruited as study group,and 42 women without PCOS received assisted reproductive technology due to male factors were enrolled as control group. The patients with PCOS were divided into overweight group（BMI≥23）and normal weight group（BMI〈23）. Plasma samples were obtained in menstrual period or anovulatory period for reproductive hormone and reactive protein C（CRP）assay, blood routine test, lipid metabolism test. Hormone assays including follicle stimulating hormone （ FSH ）, luteinizing hormone （ LH ） and testosterone （T） were detected with chemiluminescence assay. Lipid metabolism tests were executed for detecting triglyceride （TG）, low density lipoprotein （ LDI.）, high density lipoprotein （HDL）, cholesterol, apolipoprotein-A （ Apo-A ）, apolipoprotein-B（Apo-B）. Results： Leukoeytes, neutrophilic granulocytes, lymphocytes, monocytes, eosinophilic granulocytes, CRP,TG, LDL, cholesterol and Apo-B in the patients with PCOS overweight group were significantly higher than those in the normal weight group and control group,while HDL and Apo-A were significanty lower（P〈0.05）. CRP and lymphocytes were significantly higher than control group（P〈0.05）. Conclusions： The patients with PCOS suffer from chronic inflammation. The overweight patients with PCOS show increase of leukocytes, while the normal weight PCOS patients show increase of lymphocytes and abnormal lipid metabolism.
