空心钉锁定板与空心加压螺钉治疗无移位型股骨颈骨折的效果分析

目的 探讨股骨近端空心钉锁定板与空心加压螺钉治疗无移位型股骨颈骨折的效果.方法 对2009年10月-2014年10月桂林市人民医院进行手术治疗的120例Garden Ⅰ和Ⅱ型股骨颈骨折病例资料进行回顾分析.按照手术方法不同将病例分成空心加压螺钉组60例和空心钉锁定板组60例.观察术后髋关节的功能,手术时间,术中出血量,术后切口引流量,切口大小等指标.结果 空心钉锁定板组手术时间短于空心加压螺钉组(P＜0.01),切口长于空心加压螺钉组(P＜0.01);空心加压螺钉组术后髋关节功能恢复优良率高于空心钉锁定板组(P＜0.05);两组髋关节Harris评分和术后各并发症发生率比较差异无统计学意义(P＞0.05).结论 空心钉锁定板治疗无移位型股骨颈骨折具有和空心加压螺钉相当的固定强度和术后关节功能的恢复,空心钉锁定板手术切口稍长,但手术时间更短.

Effects of Cannulated Screw Locking Plate and Cannulated Compression Screw in Treatment of Non-displaced Femoral Neck Fracture

Objective To investigate effects of cannulated screw locking plate and cannulated compression screw in treatment of non-displaced femoral neck fracture.Methods Clinical data of 120 patients with femoral neck fractures (Garden type Ⅰ and Ⅱ) undergoing surgeries during October 2009 and October 2014 was retrospectively analyzed,and the patients were divided into cannulated screw locking plate group (group A,n =60) and cannulated compression screw group (group B,n =60) according to operation methods.Indexes such as postoperative hip joint function,operative time,intraoperative bleeding volume,postoperative volume of wound drainage and incision size were observed in two groups.Results In group A,operative time was shorter (P ＜ 0.01),while incision size was longer than those in group B (P ＜ 0.01).The excellent rate of postoperative hip function recovery in group B was significantly higher than that in group A (P ＜ 0.05).There were no significant differences in hip Harris score and incidence rate of postoperative complication between two groups (P ＞ 0.05).Conclusion Cannulated screw locking plate in treatment of non-displaced femoral neck fracture can achieve comparable fixed strength and postoperative recovery of joint function.The cannulated screw locking plate has a longer incision,but shorter operative time.
