安宫牛黄丸治疗热射病的疗效观察

目的观察安宫牛黄丸治疗热射病的疗效及对相关炎症因子和APACHEⅡ评分的影响。方法选择2016年6月—2020年4月沧州中西医结合医院收治的60例热射病患者,随机分为2组各30例,对照组予以西医综合治疗,观察组在此基础上联合安宫牛黄丸鼻饲治疗,均治疗7 d。观察2组肛温及意识恢复时间、治疗前后炎症因子血清肿瘤坏死因子-α(TNF-α)、白细胞介素-1(IL-1)、IL-6]水平及APACHEⅡ评分,统计2组疗效。结果观察组治疗后的肛温及意识恢复时间均明显短于对照组(P均<0.05)。观察组治疗7 d后的血清TNF-α、IL-1、IL-6水平和APACHEⅡ评分均明显低于对照组(P均<0.05)。观察组总有效率为93.3%(28/30),对照组为76.7%(23/30),2组比较差异有统计学意义(P<0.05)。结论加用安宫牛黄丸治疗热射病有助于降温、促醒,减轻炎症反应和降低APACHEⅡ评分,疗效更好。

Observation of therapeutic effect of Angong Niuhuang Pills on heat stroke

Objective It is to observe the efficacy of Angong Niuhuang Pills in the treatment of heat stroke and its influence on related inflammatory factors and APACHEⅡscore.Methods Sixty patients with heat stroke admitted to Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from June 2016 to April 2020 were selected and randomly divided into two groups,with 30 cases in each group.The control group was treated with comprehensive symptomatic and supportive treatment of western medicine,and the observation group was treated with Angong Niuhuang Pills by nasal feeding based on the control group,both groups were treated for 7 days.The rectal temperature and recovery time of consciousness,the levels of inflammatory factorsserum tumor necrosis factor-ɑ(TNF-ɑ),interleukin-1(IL-1),IL-6]and APACHEⅡscore before and after treatment were observed in the two groups,and the clinical efficacy was evaluated.Results The rectal temperature and the recovery time of consciousness in the observation group were significantly shorter than those in the control group(P<0.05).The levels of serum TNF-ɑ,IL-1,IL-6 and APACHE II scores of the observation group after 7 days of treatment were significantly lower than those of the control group(P<0.05).The total effective rate in the observation group was 93.3%(28/30)and the control group was 76.7%(23/30),the difference between the two groups was statistically significant(P<0.05).Conclusion Angong Niuhuang Pill is helpful for promoting cooling,recovery of consciousness,reducing inflammation reaction and lowering down APACHEⅡscore in the treatment of heat stroke,its curative effect is better.
