重症监护室机械通气病人肠内营养支持临床护理路径的应用研究

目的]探讨重症监护室(ICU)机械通气病人肠内营养支持临床护理路径的应用效果。方法]将60例导致呼吸衰竭而接受机械通气病人按照随机数字表法分为对照组、观察组,每组30例,对照组采用常规肠内营养支持护理模式进行护理,观察组采用ICU机械通气肠内营养支持临床护理路径模式进行护理。观察两组病人干预前后营养指标、呼吸机相关性肺炎(VAP)、胃潴留、高血糖、误吸、黏膜糜烂、腹胀、腹泻等并发症发生率情况,花费营养费用,机械通气时间,住院时间。结果]干预后观察组病人营养状况优于对照组(P<0.05);机械通气时间、住院时间、所花费的营养费用观察组均少于对照组(P<0.05);并发症发生率观察组低于对照组(P<0.05)。结论]临床护理路径在ICU机械通气病人肠内营养支持的应用效果满意,可改善病人营养状况,节约医疗资源,降低不良反应发生率。

Application of clinical nursing pathway for enteral nutrition support in ICU patients undergoing mechanical ventilation

Objective:To probe into the clinical application effect of clinical nursing pathway of enteral nutrition support in ICU patients undergoing mechanical ventilation.Methods:A total of 60 patients with respiratory failure who received mechanical ventilation were divided into control group and observation group by random number table method,30 patients in each group.Routine enteral nutrition support nursing model was used in the control group,and ICU mechanical ventilation enteral nutrition supporting clinical nursing pathway mode was used in the observation group.The nutritional indexes,incidence of complications such as ventilator-associated pneumonia(VAP),gastric retention,hyperglycemia,aspiration,mucosal erosion,bloating,diarrhea,cost of nutrition treatment,mechanical ventilation time,hospitalization days were observed before and after intervention.Results:After intervention,the nutritional status of patients in the observation group was better than that in the control group(P<0.05).The mechanical ventilation time,hospitalization days,and nutritional treatment costs of patients in the observation group were less than those in the control group(P<0.05).The incidence of complications in the observation group was less than that in control group(P<0.05).Conclusions: The clinical nursing pathway was satisfactory in enteral nutrition support for patients with ICU mechanical ventilation,which could improve the nutritional status of patients,save medical resources,and reduce the incidence of adverse reactions.
