喉部螺旋CT内窥镜的临床应用价值

摘    要:目的:探讨螺旋CT仿真内窥镜在喉部的临床应用价值。方法:共17例,其中7例为正常喉,9例为喉癌,1例为喉乳头状瘤;行喉部螺旋CT扫描后在工作站经软件处理,得到喉部仿真内窥镜图像,并与纤维喉镜进行对照研究。结果:喉部螺旋CT仿真内窥镜检查,自头端入路可见7例正常喉能清楚显示喉部解剖形态,9例喉癌和1例乳头状瘤能清楚显示肿瘤位置、形态、大小范围,与纤维喉镜检查一致。自足端入路,可见1例声门下软组织块影,2例声门下粘膜增厚,2例气管内结节影;而纤维喉镜却难以观察到。结论:螺旋CT仿真内窥镜可以清楚观察到喉腔内表面结构以及病变的位置、形态、大小范围;而足端入路对声门下结构的观察是纤维喉镜难以做到的。它是纤维喉镜的重要补充方法。

Abstract:Objective:To discuss the clinical application of spiral CT virtual endoscopy in larynx.Methods:17 cases scanned by spiral CT were studied,including 7 cases of health peoples,9 cases of laryngeal cancer and 1 case of laryngeal papillomas.The virtual endoscopy images were compared with fiber-optic laryngoscopy.Results:Spiral CT virtual endoscopy could demonstrate normal mucosal structure and mucosal lesions caranio-caudally just like the fiber-optic larynoscopy.However spiral CT endoscopy could demonstrate the lesion of subglottic region and airway by caudo-cranial approach not accessible to fiber-optic laryngoscopy.Conclusion:Spiral CT virtual endoscopy can clearly show mucousal surface structures and mucosal lesion of larynx;but it show subglottic structures by caudocranial approach not accessible to fiber-optic laryngoscopy.It is a complementary method to fiber-optic laryngoscopy.
