喉部螺旋CT内窥镜的临床应用价值

目的：探讨螺旋ＣＴ仿真内窥镜在喉部的临床应用价值。方法：共１７例，其中７例为正常喉，９例为喉癌，１例为喉乳头状瘤；行喉部螺旋ＣＴ扫描后在工作站经软件处理，得到喉部仿真内窥镜图像，并与纤维喉镜进行对照研究。结果：喉部螺旋ＣＴ仿真内窥镜检查，自头端入路可见７例正常喉能清楚显示喉部解剖形态，９例喉癌和１例乳头状瘤能清楚显示肿瘤位置、形态、大小范围，与纤维喉镜检查一致。自足端入路，可见１例声门下软组织块影，２例声门下粘膜增厚，２例气管内结节影；而纤维喉镜却难以观察到。结论：螺旋ＣＴ仿真内窥镜可以清楚观察到喉腔内表面结构以及病变的位置、形态、大小范围；而足端入路对声门下结构的观察是纤维喉镜难以做到的。它是纤维喉镜的重要补充方法。

Evaluation of Spiral CT in the Study of Larynx

Objective:To discuss the clinical application of spiral CT virtual endoscopy in larynx.Methods:17 cases scanned by spiral CT were studied,including 7 cases of health peoples,9 cases of laryngeal cancer and 1 case of laryngeal papillomas.The virtual endoscopy images were compared with fiber-optic laryngoscopy.Results:Spiral CT virtual endoscopy could demonstrate normal mucosal structure and mucosal lesions caranio-caudally just like the fiber-optic larynoscopy.However spiral CT endoscopy could demonstrate the lesion of subglottic region and airway by caudo-cranial approach not accessible to fiber-optic laryngoscopy.Conclusion:Spiral CT virtual endoscopy can clearly show mucousal surface structures and mucosal lesion of larynx;but it show subglottic structures by caudocranial approach not accessible to fiber-optic laryngoscopy.It is a complementary method to fiber-optic laryngoscopy.
