跌倒恐惧在老年终末期肾脏病患者衰弱与生活质量间的中介效应

目的 探讨跌倒恐惧及衰弱对老年ESRD患者生活质量的影响，并分析跌倒恐惧在老年ESRD患者衰弱与生活质量间的中介作用，为提高患者生活质量提供参考。方法 采用Tilburg衰弱量表(TFI)、修订版跌倒效能量表(MFES)与SF - 36生活质量量表对245名老年ESRD患者进行调查，利用SPSS 21.0对数据进行统计分析。结果 老年ESRD患者生活质量与其衰弱程度呈负相关(r = - 0.452，P＜0.01)，且跌倒恐惧水平越高，生活质量越低（r = 0.572，P＜0.01）。跌倒恐惧在衰弱与生活质量间起中介作用，中介效应占总效应的44.90%。结论 跌倒恐惧是老年ESRD患者衰弱与生活质量间的中介变量，医务人员制定干预措施时应关注患者跌倒恐惧，以期提高患者生活质量。

Fear of falling as a mediator between frailty and quality of life in elderly patients with end stage renal disease

Objective To explore the impact of fear of falling and frailty on quality of life of elderly ESRD patients and to analyze the mediating effect of fear of falling on the relationship between frailty and quality of life in elderly ESRD patients,and thus to provide reference for improving the quality of life of elderly ESRD patients. Methods 245 elderly ESRD patients were investigated with the Tilburg Frailty Indicator(TFI), Modified Fall Efficacy Scale and Short From of 36 Health Survey Questionnaire Scale. The data were statistically analyzed by SPSS 21.0. Results The quality of life of elderly ESRD patients was negatively correlated with the degree of frailty(r=-0.452, P<0.01), and the higher the level of fear of falling, the lower the quality of life(r=0.572, P<0.01). Fear of falling had mediating effect between frailty and quality of life, which accounted for 44.90% of the total effect. Conclusion Fear of falling is an intermediate variable between frailty and quality of life in elderly ESRD patients. In order to improve patients’ quality of life, clinical medical staff should pay attention to patients’ fear of falling when formulating intervention measures.
