急性泛植物神经神经病

目的　研究急性泛植物神经神经病的临床表现、诊断和治疗。方法　总结 2例患者病史、植物神经受损表现、神经电生理学及脑脊液等实验室检查特点和治疗 ,并结合文献进行分析。结果　 2例患者均急性起病 ,有明显的体位性低血压、固定心率及其他广泛的植物神经功能损害症状。结论　了解急性泛植物神经神经病的临床特点有助于正确诊断与治疗。

Acute Panautonomic Neuropathy

Objective To study the clinical manifestation, diagnosis and treatment of acute panautonomic neuropathy which is a rare disorder embraced as part of GBS. Methods We reviewed the natural history, electrophysiolgic characteristics, spectrum of autonomic involvement, and laboratory features in 2 male patients with acute panautonomic neuropathy. Results Both of the 2 patients are male who were previously quite healthy. At instant acute onset both patients had severe orthostatic hypotension to such a degree that they were unable to sit upright in bed without losing consciousness. The more striking finding is that all these 2 patients demonstrated a fixed heart rate with failure of orthostatic change and complete chronotropic incompetence. Apart from these, case 1 manifested diffuse inappropriate diaphoresis, headache, and electromyography showing evidence of peripheral neuropathy. After receiving plasma exchange the symptoms improved promptly. Case 2 demonstrated a severe and constant abdominal pain of bowel disturbance combined with fixed dilated pupils, and albuminocytologic dissociation in the spinal fluid. Conclusions Acquaintance of clinical manifestations is necessarily helpful for the diagnosis and treatment of acute panautonomic neuropathy.
