四种肿瘤标志物CYFRA21-1、NSE、CEA、CA125的联合检测在肺癌诊断中的临床价值分析研究

摘    要:【】目的:探究肿瘤标志物细胞角蛋白-19片断抗原(Cytokeratin fragment 19,CYFRA21-l)、神经元特异性烯醇化酶(Neuron-specific enolase,NSE)、癌胚抗原(Carcinoembryonic antigen,CEA)以及糖类癌抗原125(Carbohydrate antigen125,CA125)联合检测在肺癌辅助诊断中的临床价值。方法:选取我院2012年1月-2014年1月呼吸科、肿瘤科、胸外科门诊或入院的患者200例,根据病情分为分为肺癌组100例、肺良性病变组53例、健康组47例。采用电化学发光方法或酶联免疫吸附测定法S(ELISA)S检测患者血清四种肿瘤标志物CYFRA21-1、NSE、CEA、CA125的水平,并从中选取最佳的组合,评价其在肺癌诊断中的敏感度、特异度和准确度。结果:单项检测各种肿瘤标志物诊断肺癌的敏感度和准确度以CYFRA21-1最高(60.0%和83%),特异度以CA125最高(95%)。联合检测较单项检测敏感度明显增高,CEA、CA125、CYFRA21-1和NSE 四项联合效果最佳,其敏感度高达92%,特异度72%,准确度87%。结论:多项血清肿瘤标志物联合检测较单项检测明显增高诊断肺癌的敏感度和准确度。

The Analysis of Clinical Value of 4 Serum Tumor Markers CYFRA21-1, NSE, CEA, CA125 Combined Detection in Lung Carcinoma
Abstract:Objective: To explore the tumor marker cytokeratin -19 fragment antigen (Cytokeratin fragment 19, CYFRA21-l), neuron-specific enolase (Neuron-specific enolase, NSE), carcinoembryonic antigen (Carcinoembryonic antigen, CEA ) and carbohydrate cancer antigen 125 (Carbohydrate antigen125, CA125) combined detection of lung cancer diagnosis in clinical value. Methods: A hospital in January 2012 -2014 January 200 patients respiratory, oncology, thoracic surgery clinic or hospital admission, according to the disease divided into 100 cases of lung cancer, 53 cases of benign lung disease group, healthy group 47 cases. Electrochemiluminescence method or enzyme-linked immunosorbent assay (ELISA) to detect serum tumor markers in four CYFRA21-1, NSE, CEA, CA125 levels and choose from the best combination to evaluate their lung cancer diagnosis The sensitivity, specificity and accuracy. Results:The single detection of various tumor markers in patients with lung cancer with sensitivity and accuracy CYFRA21-1 highest (60.0% and 83%) and a specificity of CA125 to the highest (95%). Combined detection sensitivity was significantly higher than the single test, CEA, CA125, CYFRA21-1 and NSE four joint best, with a sensitivity of 92%, specificity 72%, accuracy 87%. Conclusion: The number of combined detection of serum tumor markers were significantly higher than the single detection sensitivity and accuracy of diagnosis of lung cancer.
