影像学在小儿肠套叠诊断和治疗中的应用

摘    要:目的:探讨影像学在小儿肠套叠诊断、治疗中的应用。方法:回顾性总结1096例肠套叠患儿B超、数字化X线机、CT及电子结肠镜影像学特点、诊断价值和治疗的效果。结果:肠套叠B超以"同心圆征"及"套筒征"为典型特征,诊断准确率99.5%。数字化X线机以"半球征"或"宫颈征"为典型特征,诊断准确率100%。二者准确率无显著性差异。CT诊断肠套叠可靠,尤其是小肠套叠更佳,电子结肠镜可以直观套叠头部。以空气为整复介质,整复率93.2%;空气灌肠失败74例均手术治愈。结论:利用B超、数字化X线机诊治肠套叠辅以手术,是儿童诊治肠套叠的主要方法。

Abstract:Objective:To study the value of imaging in the diagnosis and treatment of intussusception in children.Methods:The imaging data of 1 096cases children of intussusception on diagnosis and treatment by B-ultrasound,digitised X-ray,CT,and electronic colonoscopy were retrospectively studied.Results:The diagnosis rate of B mode ultrasonography was 99.5%in this group.The typical characteristics appeared as"multiple concentric rings"and"sleeve sign".The diagnosis rate of digital X-ray was 100%and the typical characteristics appeared as"hemisphere sign"and "cervix sign".There were no significant difference between the two groups in diagnosis rate.The reduction rate of air-enema treatment was 93.2%.All media of reduction were air.Seventy-four unsuccessful cases of air-enema treatment were all cured clinically by operation.Electronic colonoscopy could observe immediately the head of intussusception.The diagnosis of intussusception was reliable by CT,especially to the small intestine intussusception.Conclusion:The diagnosis and treatment of intussusception by B-ultrasound,digitised X-ray and open operation are the main methods in children.
