多普勒超声和心导管同步测量二尖瓣返流患者左室收缩功能的研究

本文在25例二尖瓣返流患者中,利用多普勒超声和双心导管技术同步测量了静息和负荷状态下30例次的左室收缩功能。结果显示。连续波多普勒测量的二尖瓣返流压差最大上升速率与心导管测量的左室压力最大上升速率(dp/dtmax)高度相关(r=0.87)。而脉冲波多普勒测量的升主动脉血流最大速度和平均加速度与心导管测量的dp/dtmax无显著相关(r分别为—0.18。0.07)。表明连续波多普勒超声心动图是估测二尖瓣返流患者左室dp/dtmax的可靠技术。

Quantitative Assessment of Left Ventricular Contractility in Mitral Regurgitation by Simultaneous Doppler Echocardiography and Cardiac Catheterization

In order to assess the reliability of Doppler echocardiography(DE)in the estimation of left ventricular contractility in patients with mitral regurgitation(MR),DE and left heart catheterization(Cath)were performed simultaneously in 25 patients with MR. The continuous-wave and the pulsed-wave Doppler techniques were used to record the MR regurgitant velocities and the aortic flow velocities ,respectively. The maximal change of the MR regurgitant pressure gradients with time at early systole(dPG/dt max),the peak aortic flow velocity(Vp)and the mean acceleration of aortic flow velocities(MA)were calculated from Doppler spectra. The left ventricular pressure(LVP)was recorded by Cath and the peak rate of rise of LVP(dp/dt max)was calculated. The result showed that there was a high correlation between dpG/dt max and dp/dt max(r = 0. 87),while the correlations between Vp and MA on one hand,and dp/dt max on the other,were insignificant(r = - 0. 18 and r = 0. 07,respectively), we conclude that in patients with MR Doppler derived dpG/dt max can accurately estimate dp/dt max,while Vp and MA are not good indexes for such a purpose. .
