斜裁包皮带蒂皮瓣尿道成形术治疗尿道下裂61例

摘    要:目的评价斜裁包皮带蒂皮瓣尿道成形术治疗尿道下裂的效果。方法对61例合并阴茎下弯的尿道下裂患者应用斜裁包皮带蒂皮瓣尿道成形术式,同时注意将血管蒂筋膜覆盖新形成的尿道及吻合口上面,增加尿道表面组织的厚度及血供预防尿瘘及尿道狭窄的发生。结果61例中有60例阴茎外观满意,均自正位尿道口排尿,尿道外口呈纵向裂隙状,无阴茎下弯或侧弯;并发症发生率11.5%(7/61),其中尿瘘3例、尿道外口狭窄3例,尿道口退缩1例。结论本术式具有皮瓣血运可靠、成形阴茎外观满意、取材方便、并发症少等优点,适合阴茎型、阴茎阴囊型及阴囊型尿道下裂的I期修复。

Abstract:Objective To assesse effects of urethroplasty with oblique preputial pedicled skin flap for treatment of hypospadia.Methods Sixty-one patients were performed with urethroplasty with oblique preputial pedicled skin flap.Urethra and stoma covered with blood vessel pedicle anadesma and to increase thickness and blood supply of surface tissues of ureth for prevention of development of urinary fistula and urethrostenosis.Results Out of 61 cases,60 cases showed satisfied appearance.Incidence of complication was 11.5%(7 /61),including 3 cases of urinary fistula,3 cases of urinary meatus stenosis and 1 case of urethral orifice retraction.Conclusion There are advantages including reliable blood supply of skin flap,satisfied appearance of formed cauda salax,convenient stuff,few complications,of urethroplasty with oblique preputial pedicled skin flap for treatment of hypospadia.It is an effective method to repair hypospadias of penile type and penile-scrotal type.
