盆腔神经鞘膜瘤的诊断与治疗——附3例报告

目的提高对盆腔神经鞘膜瘤诊断与治疗的认识。方法回顾性分析我院收治的3例盆腔神经鞘膜瘤患者的临床资料并复习相关文献。结果3例中,女1例,男2例,年龄36～72岁。均为体检时B超发现盆腔囊性或实质性占位,均无明显临床症状。2例查体能触及盆腔肿块。2例行CT检查,1例行MRI检查,2例经膀胱镜检查。3例均行开放手术切除肿瘤。术中证实3例肿瘤均来源于生殖股神经,打开神经鞘均可保留神经将肿瘤剥除。病理符合神经鞘膜瘤诊断。术后随访3～8个月,肿瘤无复发,无感觉及运动神经系统异常。结论膀胱周围无症状性肿瘤应考虑神经鞘膜瘤的可能性。手术切除是最好的治疗方法,手术治疗的关键点是避免损伤神经。

Diagnosis and treatment of peripheral nerve sheath tumor of the pelvic: report of three cases

Objective To improve the diagnosis and management of peripheral nerve sheath tumor of the pelvic, Methods The clinical data of three patients with peripheral nerve sheath tumor of the pelvic were analyzed retrospectively. Results Of three patients, one was female and two were male. The average age was 49.3 years, and the course of disease ranged from one to three years. The tumors of three patients were found by B-type ultrasonography. CT scan was performed in two patients, MRI in 1, and cystoscopy in two. All three patients underwent the orthotopic excision of tumors. It was found during the operations that all the tumors originated from genitofemoral nerve. After opening the nerve sheath, the tumors could be pulled off without the nerve injuried. The postoperative pathology confirmed the dignosis of peripheral nerve sheath tumor of the pelvic. During the period of follwing-up with 3-8 months, the tumor did not relapse. There were no sensory and motor nerve system disfunction. Conclusion It is should be think of the peripheral nerve sheath tumor when the mass is nearby vesical. The surgical treatment is the best choice to cure the disease, and the nerves must be protected very well during the operation.
