老年慢性酒精中毒与抑郁症的抑郁症状比较

目的 :探讨老年慢性酒精中毒性精神障碍与老年抑郁症之间抑郁症状有何差异。方法 :应用老年抑郁量表 (GDS)、汉密尔顿抑郁量表 (HAMD)、症状自评量表 (SCL 90 )及简易智能状态检查表 (MMSE)测定 4 8例伴有抑郁症状的老年慢性酒精中毒病人与 36例老年抑郁症病人。结果 :酒精中毒组的躯体症状、精神运动性抑制及自杀行为的发生率较抑郁症组低 ,其差异有显著性 (P <0 0 1) ,而焦虑与激越较抑郁症组高 ,其差异有显著性 (P <0 . 0 1)。酒精中毒组的HAMD总分明显低于抑郁症组 (P <0 . 0 1)。SCL 90中的躯体化、人际关系敏感、敌对及精神病性因子方面 ,二者之间有显著性差异 (P <0 . 0 1)。酒精中毒组MMSE总分较抑郁症组高(P <0 . 0 1)。结论 :老年慢性酒精中毒的抑郁症状程度较抑郁症轻 ,某些抑郁症状表现与抑郁症不同 ,其认知功能缺损较抑郁症为重

A Comparison of Depression Symptoms Between the Aged Chronic Alcoholic Patients and Patients with Depressive Disorder

Objective: To study the difference of depressive symptoms between the aged chronic alcoholic patients and patients with depressive disorder. Methods: 48 aged chronic alcoholic patients with depressive symptoms (AL) and 36 aged patients with depression disorder (DD) were tested with geriatric depression scale (GDS), Hamilton depression rating scale for depression (HAMD), symptom checklist 90 (SCL 90), and mini mental state examination (MMSE). Results: the rates of somatic symptoms, mental motor inhibition, and suicide behavior in AL group were lower than those in DD group (P<0 01). In AL group, comparing with DD group, the rates of anxiety and agitation were higher (P<0 01), while the total scores of HAMD were lower (P<0 01). For the test results in SCL 90, some factors (somatization, interpersonal sensitivity, depression, hostility and psychotogen) showed significant difference between the 2 groups (P<0.01). The AL group had significantly higher total scores in MMSE test than DD group (P<0.01). Conclusion: The depression degree in AL group are milder than that in DD group. Some depressive symptoms have different expression between the 2 groups. The AL group show more severe cognitive deficits.
