摘    要:目的 评价二维灰阶血流成像(B-flow)技术诊断睾丸扭转的价值.方法 选取2009年至2014年间我院经临床诊断的41例睾丸扭转患者,分别运用B-flow与高频彩色多普勒(CDFI)检测睾丸内是否存在血流信号以及血流量、分布特点,并进行对比分析.结果 41例睾丸扭转患者中,运用CDFI睾丸内血流信号检出率为39.0%(16/41),运用B-flow技术检出率为58.5%(24/41),两者比较差异有统计学意义(P<0.05),且B-flow在显示血流分布特点、血流量分级方面均优于CDFI.结论 B-flow技术在诊断睾丸扭转时在保持超声图像高帧频、高分辨率的同时能更好地检出扭转后睾丸内残存的血流,真实反映血流分布特点,评估血流量更准确,较CDFI有明显优越性,临床应用价值较高.

Application value of B-flow imaging in the diagnosis of testicular torsion
Abstract:Objective To evaluate the application value of B-flow imaging (B-flow) in the diagnosis of testicu-lar torsion. Methods Forty-one patients with testicular torsion in our hospital from 2009 to 2014 were detected by B-flow and high frequency color Doppler flow imaging (CDFI) to examine the blood flow signals, blood flow volume, and distribution characteristics, which were then compared and analyzed. Results In the 41 patients of testicular tor-sion, the detection rate of blood flow signals in spermary was 39.0% (16/41) by CDFI and 58.5% (24/41) by B-flow, with statistically significant difference (P<0.05). B-flow was better than CDFI in displaying the distribution characteris-tics of blood flow and the classification of blood flow volume. Conclusion Compared with CDFI, B-flow technique can better detect the residual blood flow of testis after torsion in the diagnosis of testicular torsion, reflect the characteris-tics of blood flow distribution and evaluate the blood flow while keeping images with high frequency and resolution, which thus has higher clinical application value.
