磁共振尿路造影对泌尿系梗阻的诊断价值

摘    要:目的:探讨磁共振尿路造影对泌尿系梗阻的诊断价值。方法:回顾性分析40例经病理、临床或随访证实的泌尿系梗阻。结果:泌尿系梗阻40例磁共振尿路造影成功率100.0%,梗阻段狭窄类型为:截断征18例,杯口状3例,充盈缺损12例,鼠尾征3例,移行性狭窄4例。MRU结合MRI定位诊断准确率达100.0%,定性准确33例(82.5%),总确诊36例(90.0%)。结论:MRU作为一种无创、安全的新技术,对泌尿系梗阻和狭窄的定位诊断具有很高的价值,结合常规MRI图像能对大多数泌尿系梗阻性疾病做出定性诊断,是IVU的有效补充检查技术。

Abstract:Objective: To study the diagnostic value of MR Urography(MRU) for urinary tract obstruction.Methods:MRI findings of urologic obstruction in 40 cases were analyzed retrospectively.All cases were confirmed by pathology,clinical practice and follow-up visit.Result:Successful rate of MRU performed in 40 patients was 100.0%.The accuracy of locative diagnosis was 100.0%(40/40),the qualitative diagnostic accuracy was 82.5%(33/44),and the general diagnostic accuracy was 90.0%(36/40) by combining MRU with conventional MRI for urologic obstructions.The shapes of obstructive extremity were variously displayed on MRI : they were cut-off-sign(18 cases),cup-port-form(3 cases),filling defect(12 cases),rat-tail sign(3 cases) and transitional narrow(4 cases).Conclusion:MRU is a new,safe and noninvasive technique,and is valuable in locating urologic obstruction.Com bining MRU with conventional MRI,we can find the cause of subtotal urologic obstructive diseases.The technique of MRU is a efficacious supplement to intravenous urography(IVU).
