经尿道等离子前列腺剜除术和电切术治疗前列腺增生的临床疗效比较

目的 探讨经尿道等离子前列腺剜除术(PKEP)和电切术(PKRP)治疗良性前列腺增生(BPH)的临床疗效.方法 将2008 年2 月至2011 年10 月在本院就诊的168 例前列腺增生患者随机均分为两组:A 组予以经尿道等离子前列腺剜除术,B 组予以尿道等离子前列腺电切术,分析两组之间手术时间,术中出血量,切除组织量,术后并发症,比较手术前后的国际前列腺症状评分(IPSS)、生活质量评分(QOL)、残余尿量(RU)、最大尿流率(Qmax)等指标,比较其临床疗效.结果 两组术后各项观察指标与术前比较,差异均有统计学意义(P<0.05),两组间切除组织量、术中出血量,手术时间比较差异有统计学意义(P<0.05).两组、术后残余尿量、IPSS 评分、生活质量评分、最大尿流率,术后并发症比较差异均无统计学意义(P>0.05).结论 两种手术方式治疗BPH 均有明显临床疗效,但PKEP 具有术中出血量少、切除组织更完全、手术时间短等优点,值得推广应用.

The observation of the cffect of plasma kinetic enucleation of prostate and plasma kinetic resection of the prostate on benign prostatic hyperplasia

Objective To analyze and observe the curative effects between bipolar plasmakinetic resection of prostate and bipolar plasmakinetic enucleation of prostate for benign prostatic hyperplasia(BPH) Methods From February 2006 to October 2011,168 examples were randomly divided into two groups:84 patients(group A)who underwent PKEP and 84 patients(group B)who received PKRP,and to analyse the operative time,resected tissue weighed,blood loss,complications,compare the differences of International Prostate Symptoms Score(IPSS),Quality of life(QOL),Residual urine(RU)and maximum urine flow rate(Qmax)pre-and postop-eratively.Results All indexes before and after surgery had statistically significant difference(P<0.05).Blood loss,resected tissue weighed and the operative time were significant differences between the 2 groups(P<0.05).The postoperative RUV,IPSS,QOL,Qmax and complications were no significant differences between the 2 groups(P>0.05).Conclusions Two methods for the treatment of BPH is reliable effect,but the blood loss and incidences of contemporary incontinence of PKER have more superiority than those of PKRP,so PKER deserves our application.
