早期胃癌患者术前胃镜病理活检与术后病理结果的一致性分析

目的探讨早期胃癌患者术前胃镜活检与术后病理结果的一致性。方法回顾性分析我院2015年1月至2018年6月收治的72例疑似早期胃癌患者的术前胃镜活检与术后病理结果资料,分析并比较两种方法的诊断结果、病理类型及分化程度检出率。结果术前胃镜活检的准确度为91.67%,灵敏度为93.10%,特异度为85.71%。两种方法对乳头状腺癌、粘液腺癌、管状腺癌、印戎细胞癌及混合性腺癌的检出率比较,差异无统计学意义(P>0.05)。术前胃镜活检的分化不良型检出率明显高于术后病理检查(P<0.05)。结论术前胃镜活检虽不能对早期胃癌的分化程度及病理类型进行精准判断,但其准确度、灵敏度及特异度均较高,在胃癌的预防及早期诊疗中具有积极作用。

Consistency analysis between preoperative gastroscopic biopsy and postoperative pathological results in patients with early gastric cancer

Objective To investigate the consistency between preoperative gastroscopic biopsy and postoperative pathological results in patients with early gastric cancer. Methods A retrospective analysis was made for data of the preoperative gastroscopic biopsy and postoperative pathological results in 72 suspected early gastric cancer patients admitted in our hospital from January 2015 to June 2018. The diagnostic results, detection rates of pathological types and differentiation of the two methods were analyzed and compared. Results The accuracy, sensitivity and specificity of preoperative gastroscopic biopsy were 91.67%, 93.10% and 85.71% respectively. There were no significant differences in the detection rates of papillary adenocarcinoma, mucinous adenocarcinoma, tubular adenocarcinoma, signet-ring cell carcinoma and mixed adenocarcinoma between the two methods(P>0.05). The detection rate of poorly differentiated type by preoperative gastroscopic biopsy was significantly higher than that by postoperative pathological examination(P <0.05).Conclusion Preoperative gastroscopic biopsy can not accurately judge the differentiation degree and pathological type of early gastric cancer, but it has high accuracy, sensitivity and specificity. It plays an active role in the prevention and early diagnosis and treatment of gastric cancer.
