脑血栓形成患者体外反搏后血浆t－PA，PAI，D－dimer的变化及意义

了解体外反搏治疗对脑血栓患者的疗效及作用机制。用酶联免疫吸附法（ＥＬＩＳＡ）和比色分析法分别测定了２０例脑血栓形成患者和１０例非脑血栓形成患者体外反搏前后血浆组织型纤溶酶原激活剂（ｔｉｓｓｕｅｐｌａｓｍｉｎｏｇｅｎａｃｔｉｖａｔｏｒ，ｔ－ＰＡ）和纤溶酶原激活剂抑制物（ｐｌａｓｍｉｎｏｇａｃｔｉｖａｔｏｒｉｎｈｉｂｉｔｏｒ，ＰＡＩ）活性及Ｄ－二聚体（Ｄ－ｄｉｍｅｒ）含量的变化。结果：检测到脑血栓形成患者在体外反博（ＥＣＰ）后，血浆ｔ－ＰＡ活性增高伴有Ｄ－ｄｉｍｅｒ含量增加；在非脑血栓形成患者有血浆ｔ－ｐＡ活性增高无Ｄ－ｄｉｍｅｒ含量变化。ＰＡＩ的活性于两组中均无变化。结论：体外反搏治疗后，脑血栓患者血浆中ｔ－ＰＡ水平升高，并可使纤维血块溶解。

ALTERNATIONS AND SIGNIFICANCE IN PLASMA LEVELS OF TISSUE PLASMINOGEN ACTIVATOR AND ITS INHIBITOR AND D-DIMER AFTER EXTERNAL COUNTERPULSATION IN ATHEROTHROMBOTIC BRAIN INFARCTION

PURPOSE Since venous occlusion can increase fibrinolytic capacity,we study the relationship between the thrombotic disease and external counterpulsation in order to understanding its therapeutic effect and mechanism.METHODS The plasma levels of tissue plasminogen activator(t-PA)and plasminogen activator inhibitor(PAI)activity as well as D-dimer antigen were determined in 20 atherothrombotic brain infarction patients and 10 patients without atherothrombotic brain infarction by the enzyme linked immunosorbent assay(ELISA)and the spectrophotometric assays,respectively.RESULTS It was found that the plasma leveles of t-PA and D-dimer were significantly increased after external counterpulsation in atherothrombotic brain infarction patients while no significant increase of D-dimer in patients without atherothrombotic brain infarction. There was no significant change of PAI in both groups.CONCLUSIONS External counterpulsation increase plasma leveles of t-PA in athero thrombotic brain infarction patients and cause fibrin clot to be dissolved.
