艾司西酞普兰与帕罗西汀治疗老年脑卒中后抑郁的对照研究

目的研究艾司西酞普兰治疗老年脑卒中后抑郁的临床疗效及安全性。方法将80例老年脑卒中后抑郁症患者随机分为两组,艾司西酞普兰治疗组40例,帕罗西汀治疗组40例,疗程8周。用汉密尔顿抑郁量表(HAMD)评估患者疗效。用副反应量表(TESS)评估患者的药物不良反应。结果在第1周末时艾司西酞普兰组HAMD总分低于帕罗西汀组(t=4.07,P<0.01)。在治疗8周末,艾司西酞普兰组有效率75.0%,对照组有效率为72.5%,两组有效率比较差异无统计学意义(P>0.05)。且艾司西酞普兰组不良反应率低于帕罗西汀组。结论艾司西酞普兰与帕罗西汀治疗老年脑卒中后抑郁疗效相当,但艾司西酞普兰起效快,依从性好,不良反应轻微,值得推广。

Comparative Study of Escitalopram and Paroxetine in the Treatment of the Elderly Post - stroke Depression

Objective To compare the efficacy and security of Escitalopram and Paroxitine in the treatment of the elderly post-stroke depression.Methods A total of 80 elder patients with post-stroke depression were randomly divided into two groups which received Escitalopram(n=40) or Paroxitine(n=40) for 8 weeks.The efficacy was evaluated with Hamilton Depressive Scale(HAMD).The Treatment Emergent Symptom Scale(TESS) was used to evaluate the side effects.Results The scores of HAMD decreased significantly at the end of the first week in Escitalopram group,but did not in the Sertraline group(t=4.07,P<0.01).The effective rate was 75.0% and 72.5% in escitalopram group and in paroxetine group respectively.There was no statistical difference on the effective rate between the two groups(P>0.05).The rate of adverse reactions in escitalopram group was lower than that in paroxetine group. Conclusion Escitalopram and Paroxitine are effective drug in the treatment of elderly post-stroke depression,but Escitalopram takes effect more quickly,has better compliance and mild adverse reactions.It is worth popularizing.
